Experience of double valve replacement with St. Jude medical prosthesis

Abstract
OBJECT: Evaluation of early and long term results of double valve replacement with St. Jude medical mechanical heart valve prosthesis and a standard fixed dose oral anticoagulant therapy retrospectively. METHOD: During 1986-1996, for a ten year period, 489 patients underwent double valve replacement with St. Jude medical heart valve prosthesis in Türkiye Yüksek ihtisas Hospital, Cardiovascular Surgery Clinic. The mean age of the patients were 39.7±12.6 (9-80). The mean follow-up period was 2.9±2.4 patient year (1-126.8 months). Evaluation of the patient population was performed in retrospective manner. RESULTS: Hospital (early-operative) mortality was 6.1% (30 patients). In the follow-up period, there were 13 thromboembolic events, and estimate of freedom from thromboembolic events at 10 year was 91.5±2.6%. Fifteen patients developed mechanical valve thrombosis. Estimate of freedom from mechanical valve thrombosis at ten year was 91.2±1.5%. There were 14 oral anticoagulant related hemorrhage, and estimate of freedom from oral anticoagulant related hemorrhage at 10 year was 72.9+10%. There were 4 paravalvular leak, and estimate of freedom from paravalvular leak, at 10 year was 98.6+0.8%. A total of 23 patients were reoperated and estimate of freedom from reoperation at 10 year was 80.6+5.3%. In the follow-up period, there were 12 late deaths and the linearised late mortality rate was 0.9%ppy Estimate of actuarial survival at 10 year was 96.3±1.2%. According to Cox statistical analysis, female gender and patient age here statistically significant operative mortality risk factors for double valve replacement (p<0.01). CONCLUSION: Double valve replacement with St. Jude medical mechanical heart valve prosthesis has excellent results although a standard fixed dose oral anticoagulant therapy was used.

AMAÇ: St. jude medikal mekanik kapakla çift kapak replasmanı ve standart fiks doz OAK tedavisinin erken ve geç dönem sonuçları retrospektif olarak araştırılmıştır. YÖNTEM: Türkiye Yüksek İhtisas Hastanesi, Kardiyovasküler Cerrahi kliniğinde, 1986-1996 yılları arasında, St. Jude mekanik kapak protezi ile 489 hastaya çift kapak replasmanı yapıldı. Hastaların ortalama yaşı 39.7±12.6 (9-80) idi. Tüm hastalara standart doz oral antikoagülan tedavi başlandı. Ortalama takip süresi 2.9±2.4 olarak bulundu (1-126.8 ay). BULGULAR: Hastane (erken-operatif) mortalitesi %6.1 olarak bulundu (30 hasta). Geç dönemde, 13 hastada tromboembolizm komplikasyon gelişti. 10 yıllık tromboembolik komplikasyonsuz yaşam oram, %91.5±2.6 idi. 15 hastada mekanik kapak trombozu tespit edildi. 10 yıllık mekanik kapak trombozsuz yaşam oranı %91.2±1.5 idi. 14 hastada oral antikoagülana bağlı kanama görüldü. 10 yıllık oral antikoagülana bağlı kanama olmaksızın yaşama oranı %72.9±10 idi. Paravalvüler kaçak 4 hastada gözlendi. 10 yıllık paravalvüler kaçaksız yaşama oranı %98.6±0.8 olarak bulundu. Takip periyodu içerisinde 23 hasta reopere edildi. 10 yıllık reoperasyonsuz yaşam oranı %80.6±5.3 idi. Takip periyodunda 12 ölüm ile linearize geç mortalite hızı; %0.9 hastaydı olarak bulunmuştur. 10 yıllık yaşam, %96.3±1.2 idi. Cox analizine göre hasta yaşı ve bayan cinsi çift kapak replasmanı için aperatif mortalite faktörü olarak istatistiki anlama sahip bulundu (p<0.01). SONUÇ: St. Jude medikal mekanik kapakla çift kapak replasmanı gerçekleştirilmiş standart fiks doz OAK tedavisi kullanılan hasta populasyonunda, erken ve geç dönemde mortalite ve morbidite açısından başarılı sonuçlar elde edilmiştir.
