Platelet function parameters in patients with complicated acute myocardial infarction

Abstract

AIM: This study was designed to assess the relationship between complications of acute myocardial infarction (AMI) and platelet function parameters. MATERIAL AND METOD: In order to investigate the variations in some platelet function parameters AMI 52 patients (39 males, 13 females) with the diagnosis of AMI and applying for the first 24 hours after symptoms occured were enrolled into this study. Patients who developed severe arrhythmia, clinically relevant hearth failure, cardiogenic shock, reinfarction, and patients who died as a consequence of AMI were accepted as complicated cases. According to complications the patients with AMI were subdivided into two groups as complicated cases (n=22, group I) and uncomplicated cases (n=30, group II). Twenty healthy volunteers were taken as the controls. To investigate the variations in some platelet function parameters circulating aggregated platelets (total, reversibly and irreversibly aggregated), the number of platelets per aggregate and big platelets were measured by a modification of the Wu and Hoak method on the first, second and fifth day of AMI. Additionally, platelet numbers and mean platelet volumes were determined. RESULTS: In comparison of group I with group II, there were significant differences especially on the second day of AMI, in the values of total aggregated platelets (36.73±4,62 % vs 25.14±4.48%, p<0.001), reversibly aggregated platelets (28.39±4.70% vs 14.67±4.78%, p<0.0001), irreversibly aggregated platelets (8.39±2.63% vs 10.42±1.85%, p<0,05) and the average platelet per aggregate (8.13±1.31 vs 4.43±l.45, p<0,0001). While mean platelet volumes both in group I and group II were found to be significantly higher when comparated with the control group (p<0.001), there was no significant difference between group I with group II (p>0.05). CONCLUSION: These findings demonstrate that, the variations in some platelet function parameters might play an important role not only in the occurence of AMI but also in the development of complications.

AMAÇ: Bu çalışma trombosit fonksiyonları ile akut miyokard infarktüsü (AMI) 'nün oluşması ve AMI'ne bağlı komplikasyonların gelişimi arasındaki ilişkiyi araştırmak amacı ile yapıldı. GEREÇ VE YÖNTEM: Trombosit fonksiyon parametrelerindeki değişiklikleri araştırmak için AMI tanısı konan ve semptomların başlamasından sonra ilk 24 saat içinde başvuran 52 hasta (39 erkek, 13 kadın) alındı. Ciddi aritmi, klinik olarak belirgin kalp yetmezliği, kardiyojenik şok, reinfarktüs gelişen ve AMI sonucu ölen hastalar komplikasyon gelişen olgular olarak kabul edildi. AMI'lü hastalar komplikasyonların olup olmamasına göre komplikasyon gelişen olgular (n=22, grup I) ve komplikasyon gelişmeyen olgular (n=30, grup II) olarak iki alt gruba ayrıldı. Yirmi sağlıklı gönüllü kontrol grubu olarak alındı. Bazı trombosit fonksiyon parametrelerindeki değişiklikleri araştırmak için Wu ve Hoak 'un modifiye bir yöntemi kullanılarak AMI' nün 1., 2. ve 5. günlerinde dolaşan agrege olmuş trombositler (total, reversibl ve irreversibl agrege olmuş trombositler), her agregattaki trombosit sayısı ve büyük trombositler ölçüldü. İlave olarak ortalama trombosit hacmi ve trombosit sayısı belirlendi. BULGULAR: Grup I ile grup II karşılaştırıldığında; özellikle AMI'nün 2. gününde total agrege olan trombositler (%36.73±4.62 ve %25.14±4.48, p 0.05). SONUÇ: Bu bulgular bazı trombosit fonksiyon parametrelerindeki değişikliklerin AMI' nün sadece oluşmasında değil aynı zamanda komplikasyonlann gelişmesinde de rol oynayabileceğini göstermektedir.
