Postoperative complications in cardiac surgery and CRP

Abstract
BACKGRAUND: It is aimed to search the role of C-reactive protein (CRP) in the patients who are applied the open cardiac surgery developing the postoperative complications which is determined before. MATERIAL AND METHODS: In Erciyes University Medical Faculty Department of Thoracic and Cardiovascular Surgery between 1st March 1998-1st November 1998 in 69 patients who had an operation, the preoperative and postoperative serum CRP levels are evaluated. RESULTS: Major complication emerge of 8 cases of 3 (11.6%) has mortal situation. When compared the preoperative CRP values of these patients with the patients who has not complication CRP values, there is not a real difference. The patients who did not emerge the complication, the postoperative CRP concentrations are increasing rapidly after the second day. It reaches the peak level and later on it decreases steadily. On the contrary, the complication developing patients of all, in the postoperative period the CRP levels whether increases always or after decreasing it increases for the second time. Yet, the CRP level in the recovering patients begins decreasing later. CONCLUSIONS: The emerging results shows that the preoperative CRP values before determining the complications have not a real role and puts forward the postoperative serial CRP values can be a useful method.

AMAÇ: Açık kalp cerrahisi uygulanan hastalarda gelişen postoperatif komplikasyonların önceden belirlenmesinde C-reaktif proteinin (CRP) rolünün araştırılması. GEREÇ VE YÖNTEM: Erciyes Üniversitesi Tıp Fakültesi Göğüs Kalp ve Damar Cerrahisi Anabilim Dalında 1 Mart 1998- 1 Kasım 1998 tarihleri arasında ameliyat edilen 69 hastada preoperatif ve postoperatif serum CRP değerleri ölçüldü. BULGULAR: Majör komplikasyon ortaya çıkan sekiz olgunun (%11.6) üçü mortal seyretti. Bu hastaların preoperatif CRP değerleri, komplikasyon ortaya çıkmayan hastaların CRP değerleriyle karşılaştırıldığında anlamlı bir fark yoktu. Komplikasyon ortaya çıkmayan hastaların postoperatif CRP konsantrasyonları hızlı bir yükselişi takiben 2. günde peak seviyesine ulaşıyor ve daha sonra yavaş yavaş düşüyordu. Buna karşılık komplikasyon gelişen hastaların tümünde postoperatif dönemde CRP seviyeleri ya devamlı yüksek seyrediyor veya düşmeye başladıktan sonra ikinci bir yükseliş gösteriyordu. Ancak iyileşen olgularda CRP düzeyi daha sonraları düşmeye başlıyordu. SONUÇ: Elde edilen bulgular komplikasyonların önceden belirlenmesinde preoperatif CRP ölçümlerinin anlamlı bir rolü olmadığını düşündürürken, postoperatif seri CRP ölçümlerinin ise kullanılabilecek bir yöntem olduğunu ortaya koymaktadır.
