Evaluation of myocardial perfusion by Tc-99m tetrofosmin SPECT before and after PTCA and intracoronary stents

Abstract
AIM: Tc-99m tetrofosmin, a relatively new myocardial imaging agent, has shown encouraging results in the detection of myocardial ischemia. The aim of the study was to evaluate the usefulness of Tc-99m tetrofosmin in demonstrating ischemia before coronary interventions and in assessing success of mechanical revascularization achieved by PTCA or intracoronary stent placement. METHOD: Twenty-nine patients undergoing mechanical revascularization achieved by PTCA or intracoronary stent placement were enrolled to this study. All patients underwent Tc-99m tetrofosmin SPECT myocardial perfusion scintigraphy within 1 week before and I month after revascularization. Tc-99m tetrofosmin myocardial tomograms were analyzed by semiquantitative segmentary visual assessment. RESULTS: Before revascularization exercise ECG and Tc-99m tetrofosmin scintigraphy were abnormal in 14 (48%) and in 28 (96%) of the patients, respectively. The average rate pressure-product at peak exercise was 232.92±41.19 (x100). Ischemia was detected in 101 myocardial segments out of 186 segments supplied with 29 stenotic coronary arteries in which intervention was performed before the procedure. Total exercise perfusion score was 311 in 29 patients before revascularization. One month after the revascularization the number of patients with an abnormal exercise ECG decreased from 14 to 2 after the intervention. The average rate-pressure product increased from 232.92±41.19 to 271.27±35.44 (x100) (p<0.05). The number of patients with an abnormal Tc-99m tetrofosmin scintigrapy decreased from 28 to 5 after the procedure. After revascularization, the number of ischemic segments significantly decreased from 101 to 9 (p<0.001). Exercise perfusion total scores significantly improved after revascularization from 317 to 67(p<0.001). CONCLUSION: Tc-99m tetrofosmin myocardial perfusion scintigraphy, having high quality imaging properties, is a very useful method to demonstrate ischemia and jeopardized myocardium before intervention and to demonstrate improvement of myocardial perfusion and the success of the intervention after coronary interventions.

AMAÇ: Tc-99m tetrofosmin rutin kullanıma girmiş yeni bir myokard perfüzyon sintigrafisi ajanı olup yapılan çalışmalarda myokard perfüzyon değişikliklerini göstermede oldukça başarılı olarak bulunmuştur. Bu çalışmanın amacı, mekanik revaskülarizasyon (perkütan translüminal koroner anjiyoplasti ve intrakoroner stent implantasyonu) uygulanan hastalarda işlem öncesinde iskeminin ve risk altındaki myokardın gösterilmesi ve işlem sonrasında myokard perfüzyonundaki iyileşmenin gösterilmesinde Tc-99m tetrofosminin yerini değerlendirmektir. YÖNTEM: Çalışmaya perkütan translüminal koroner anjiyoplasti (PTCA) veya intrakoroner stent implantasyonu şeklinde mekanik revaskülarizasyon uygulanan 29 hasta alındı. Bütün hastalara revaskülarizasyondan 1 hafta önce ve 1 ay sonra Tc-9m tetrofosmin SPECT (Single Photon Emission Computed Tomography) myokard perfüzyon sintigrafisi çalışması yapıldı. Görüntüler semikantitatif vizüel skorlama ile değerlendirildi. BULGULAR: Revaskülarizasyon öncesi egzersiz EKG 14 (%48) hastada, Tc-99m tetrofosmin SPECT 28 (%96) hastada anormal olarak bulundu. "Double Product" değeri işlem öncesi 232.92±41.19 (x100) olarak bulundu. İskemi, mekanik revaskülarizasyonun gerçekleştirildiği 29 koroner arterin beslediği 186 myokard segmentinin 101'inde saptandı. 29 hastada işlem öncesi egzersiz perfüzyon skoru 311 idi. İşlem öncesi 14 olan anormal egzersiz EKG sayısı revaskülarizasyondan 1 ay sonra 2 hastaya düştü. Ortalama "double product" değeri işlem öncesi 232.92±41.19 (x100)'dan işlem sonrası 271.27±35.44 (x100) değerine ulaştı (p<0.05). İşlem öncesi sintigrafide perfüzyon bozukluğu 28 hasta iken işlem sonrası bu sayı 5'e indi. Revaskülarizasyon sonrası, iskemik segment sayısı anlamlı olarak azalarak 101'den 9'a düştü (p<0.001). SONUÇ: Tc-99m tetrofosmin myokard perfüzyon sintigrafisinin, mekanik revaskülarizasyon uygulanan hastalarda yüksek görüntü kalitesiyle işlem öncesi iskeminin ve risk altındaki myokardın, işlem sonrasında ise myokard perfüzyonundaki iyileşmenin ve işlemin başarısının gösterilmesinde başarılı bir sintigrafik teknik olduğu sonucuna varılmıştır.
