The role of tranexamic acid in postoperative bleeding and effects according to the variaties of its usage

Abstract

PURPOSE: Tranexamic acid (T. I) has been shown to decrease bleeding in CPB although the ideal administration regimen is still not clear. In this randomised controlled clinical study, TA infusion was compared with bolus administration in terms of postoperative bleeding and transfusion requirements. METHODS: 45 adult patients undergoing elective CABG were enrolled in the study. Group I (n=I5) control group, received placebo infusion of saline, group II (n=15) received tranexamic acid; before skin incision (15 mg/kg) and after CPB (15 mg/kg bolus) , group III (n=15) received a bolus dose of tranexamic acid followed by a 1 mg/kg infusion for 6 hrs. Recorded parameters were, ACT, haemoglobin (Hb), haemotocrit (Htc), platelet count, volume of shed blood through the drains, fibrinogen and transfusion requirements. Data were recorded after protamine administration and at postoperative 6,12 and 24th hour. RESULTS: ACT was higher in group II than III at 6th hour postoperatively (p< 0.05). Hb level was higher in group III than I at postoperative 24th hour (p< 0.05). Shed blood through the drains in 24th hr was less in Group II (p<0.05) and III (p<0.05) than I. Fibrinogen levels were lower after CPB and at 6 and 24th hour postoperatively in Group III than I (p<0.05), but the mean values were within the normal laboratory range. Mean units of blood and fresh frozen plasma transfusion was less in Group III than I and II (p<0.05). CONCLUSION: Although bolus administration of T A is effective in decreasing bleeding compared to placebo, continous infusion regimen is superior in decreasing blood loss and reducing the requirement for transfusions.

AMAÇ:Kardiyopulmoner bypass (KPB) uygulanan hastalarda traneksamik asitin kanamayı azalttığı saptanmış, ancak ideal uygulama şekli ve dozu kesin olarak belirlenmemiştir. Bu randomize kontrollü çalışmada traneksamik asit infüzyonu ile bolus uygulaması postoperatif kumuna, transfüzyon gereksinimi ve hematolojik parametrelere etkileri açısından karşılaştırıldı. YÖNTEM: Elektif koroner arter cerrahisi planlanan 45 erişkin hasta etik komite onayını izleyerek çalışma grubuna alındı. Grup I (n 15) kontrol grubuna plasebo salin infüzyonu uygulandı. Grup II'deki (n 15) hastalara cilt insizyonundan önce ve KPB sonunda iki kez bolus doz şeklinde 15 mg kg traneksamik asit uygulandı. Grup III'deki (n 15) hastalara cilt insizyonundan önce ve KPB sonunda iki kez bolus doz şeklinde 15 mg kg bolus TA 'yi takiben 6 saat süre ile 1 mg. kg saat hızda infüzyon uygulandı. Çalışma süresince; protamin uygulamasını izleyerek ve postoperatif 6, 12 ve 24. saatte ölçülen ve kaydedilen parametreler; ACT, hemoglobin (Hb), hematokrit (Htc), trombosit sayısı, drenlerden biriken kanın hacmi, fibrinojen ve transfüzyon miktarı idi. BULGULAR.-Postoperatif 6. saatte ACT Grup II'de Grup III'den daha yüksek bulundu (p< 0.05). Postoperatif 24. saatte Hb Grup III'de Grup I'den daha yüksek idi (p< 0.05). 24 saatlik drenaj miktarı Grup II (p< 0.05) ve Grup III'de (p< 0.01) Grup I'den daha az idi. Fibrinojen düzeyi protamin uygulamasını izleyerek ve postoperatif 6 ve 24. saatte Grup III'de Grup I'den daha düşük bulundu (p< 0.05), ancak ortalama değerler normal sınırlar içerisindeydi. Ortalama kan ve taze donmuş plazma transfüzyon miktarı Grup III'de Grup I ve Grup II 'den daha az idi (p< 0.05). SONUÇ:Bu çalışmada bolus traneksamik asitin uygulanmasının kontrol grubu ile karşılaştırıldığında kanama miktarını azalttığı saptanmıştır, ancak traneksamik asit infüzyonunun post operatif kanama miktarının ve kan transfüzyonu gereksiniminin azaltılmasında hem kontrol hem de bolus uygulamadan daha etkili olduğu bulunmuştur.
