The HBsAg positivity in children with congenital heart disease (CHD) before cardiac catheterization

Abstract

AIM: The aim of this study to determine the hepatitis B markers in children with congenital heart disease (CHD) before cardiac catheterization and, to compare their hepatitis B virus (HB V) carriage and exposure rates with those of general population. METHOD: One hundred ten children (56 girls, 54 boys) with CHD were tested for hepatitis B markers (HBsAg, Anti-HBsAb, Anti-HBcAb, HBeAg, Anti-HBeAb) in order to determine the carriage of HbsAg with the other routine pre-catheter tests before diagnostic or interventional cardiac catheterization between January and December 1994. RESULTS: Two of the 110 children (1.8 %) were found to be positive for HBsAg, 1 (0.9 %) to HBsAg and Anti-HBeAb, 1 (0.9 %) to Anti-HBsAb and none were found positive for Anti-HBcAb and HBeAg. Carriage of HBsAg 2,7 %, the hepatitis B virus exposure rate 3,6 % were found. DISCUSSION: Although the carriage of HBsAg and the HBV exposure rate of our study group were lower than those of the general population, it is prudent that all patients who undergo invasive procedures should be tested for HBV seromarkers as a routine with regard to health workers and medical instruments. Since the most mode of transmission of HBV infection was contaminated blood, the health workers should be called attention for the HBsAg positive patients with CHD. They should be included to vaccination program against HBV for HBsAg negative patients with CHD in order to protect for probable contamination in future.

AMAÇ: Bu çalışmada, konjenital kalp hastalığı (KKH) olan çocukların kardiyak kateterizasyon öncesi hepatit B virus (HBV) belirteçlerini denetleyerek taşıyıcılık, HBV ile karşılaşma sıklığı yönünden toplum geneli ile karşılaştırılması amaçlanmıştır. METOD: Ocak-Aralık 1994 tarihleri arasında tanı veya tedavi amacıyla yapılan 110 kardiyak kateterizasyon vakasının (56 kız, 54 erkek) diğer rutin tetkikler ile birlikte HBV belirteçleri (HBsAg, Anti-HBsAb, Anti-HBcAb, HBeAg, Anti-HBeAb) uygulama öncesi ilene 11 endi. SONUÇLAR: Ailede ve öz geçmişlerinde hepatit öyküsü olmayan vakalardan 2'sinde HBsAg pozitif (% 1.8), birinde HBsAg ile birlikte Anti-HBeAb pozitif (% 0.9), bir vakada da Anti-HBsAb'in pozitif (% 0,9) olduğu görüldü. Ant i-HBe Ab ve HbeAg pozitif olan vaka yoktu. HBsAg pozitifliği % 2,7, HBV ile karşılaşma sıklığı % 3,6 olarak saptandı. TARTIŞMA: Yüzey antijen HBsAg taşıyıcılığı ve HBV ile karşılaşma sıklığı genel toplumda görülenden daha yüksek değildi. Bizim hastane prensiplerimiz içinde bu grup genel popülasyondan farklı olmasa da tüm invaziv işlev yapılacak hastalara HBV göstergelerinin bakılması malzeme ve personel açısından önemlidir. Bu nedenle bütün invaziv tetkik yapılacak olgulara bu testler yapılmalıdır. Uygulanan tekrarlı kateter, açık kalp ameliyatı gibi işlemler HBV enfeksiyonunun başlıca geçiş nedeni olan bulaşık kanla temas riskini arttırmaktadır. Sonuçta KKH olan çocukların risk grubu olarak kabul edilip HBV belirteçlerine bakılmaksızın aşılama programına alınmaları güvenli ve ekonomik olacaktır.
