A case report of traumatic reno-caval fistula

Abstract
Arteriovenous fistulas are an abnormal connections between a high-pressure, high- resistance arterial system and low-pressure, low-resistance, high-capacity venous system. Because of the low resistance, blood preferentially flows through the fistula rather than through the normal capillary bed. The amount of blood shunted depends on the diameter of the fistula and its proximity to the heart. As a result of increased circulating total blood volume, there is a progressive dilatation of the entire circulatory system proximal to the fistula. Cardiac enlargement and venous distantion may reverse after the fistula closure. in this report, following a stab wound, a case with traumatic arteriovenous fistula between the right renal artery and the inferior vena cava that occurs signs of heart failure is presented and reviewed traumatic arteriovenous fistulas.
Özet
Arteriyovenöz fistüller yüksek basınçlı ve yüksek rezistanslı arteriyel sistem ile düşük basınç, düşük rezistans ve yüksek kapasitansa sahip venöz sistem arasındaki anormal bağlantılardır. Bu düşük rezistans nedeniyle, kan normal kapiller yataktan ziyade fistül aracılığı ile akmayı tercih eder. Şant miktarı fistülün çapma ve kalbe olan yakınlığına bağlıdır. Dolaşan total kan volümünün artması sonucunda fistül proksimalinde yer alan tüm dolaşım sisteminde progresif dilatasyon gelişir. Kardiyomegali ve venöz distansiyon fistülün kapatılmasından sonra düzelebilir. Bu makalede bıçaklanmayı izleyerek kalp yetmezliği bulgularıyla ortaya çıkan sağ renal arter ile inferior vena cava arasında travmatik arteriovenöz fistüllü bir olgu sunulmuş ve travmatik arteriyovenöz fistüller gözden geçirilmiştir.
