Gastrointestinal complications after cardiac surgery

Abstract

AIM: Gastrointestinal complications are not very common after cardiac surgery. But if exists morbidity and mortality rates are very high. 1455 cardiac surgical patients were investigated due to gastrointestinal complications incidence, risk factors and mortality. MATERIAL AND METHODS: Between January 1990 and December 2000. Most common gastrointestinal complications after cardiac surgery were; pancreatitis, gastritis, laparatomy, gastric ulcer, gastric and bowel perforation, gastrointestinal bleeding, diverticulitis, ileus and visceral ischemia. There were nine major gastrointestinal complications (0.6%). Three of these cases were died as a mortality rate of 33.3%. These complications were; 4 bleeding (44.4%), 2 acute cholesistitis (22.2%), l acute pancreatitis (11.1%), l bowel necrosis (11.1%) and l iatrogenic gastric perforation (11.1%). RESULTS: Four cases had gastrointestinal complainments preoperatively (p< 0.001). Three were female (p<0.05), mean age was 63±9.7 (p<0.05), cardiopulmonary bypass time was 124.2+30.1 min (p<0.001) and cross-clamping time was 84±23.8 min (p>0.05). CONCLUSION: Advanced age, CBP time, cross damping time, female gender and previous gastrointestinal complainments are closely related with these complications. Prophylaxis and early diagnosis and treatment are very important for these gastrointestinal complications.

AMAÇ: Gastrointestinal komplikasyonlar kardiyak cerrahi sonrası oldukça nadirdir. Fakat oluştuğunda morbidite ve mortalitesi yüksektir GEREÇ VE YÖNTEM: Ocak 1990 - Aralık 2000 tarihleri arasında kardiyopulmoner bypass uyguladığımız 1455 olguda majör gastrointestinal komplikasyonların insidansı, risk faktörleri ve mortalitesi incelendi. Pankreatit, gastrit, laparatomi, gastrik ülser, kolesistit, mide-bağırsak perforasyonu, gastrointestinal kanama, divertikulit, ileus ve visseral iskemi kardiyopulmoner bypass sonrası görülebilen majör komplikasyonlardır, BULGULAR: Olgularımızın dokuzunda (%0,6) majör gastrointestinal komplikasyon görüldü. Üst gastrointestinal kanama dört (%44,4) olguda, akut kolesistit iki (%22,2) olguda, akut pankreatit bir (%11,1) olguda, barsak nekrozu bir (%11,1) olguda ve iatrojenik mide perforasyonu bir (%11,1) olguda görüldü. Bu olgulardan 3'ü (%33,3) kaybedildi. Majör gastrointestinal komplikasyonlu olgularımızın dördünde preoperatif gastrointestinal şikayet (p 0.05) olduğu tespit edildi. SONUÇ: Risk faktörleri (gastrointestinal şikayet, kadın cinsiyet, ileri yaş ve uzun kardiopulmoner bypass) bulunan olgularda gerek proflaktik tedavi, gerekse erken tam ve tedavi son derece önemlidir.
