Comparison of sotalol with digoxin-quinidin for conversion of acute atrial fibrillation to sinus rhythm

Abstract
Comparision of Sotalol With Digoxin-Quinidin for Conversion of Acute Atrial Fibrillation to Sinus Rhythm OBJECTIVE: The aim of this study is to evaluate the effectiveness of sotalol which is a class III antiarrhythmic agent with betablocker affects, on the conversion of paroxysmal atrial fibrillation (AF) episodes to sinus rhythm, and to compare it with the effect of digoxin-quinidine. METHODS: 40 patients with paroxysmal AF (<48 hours from onset) were included into this prospective randomized study. All patients had >80 beats/min heart rate and >120 mmHg systolic blood pressure. Patients were divided into randomized two groups according to the treatment regime. Sotalol of 160 mg was given orally to group I patients (13 male, 6 female; mean age, 53.1 ±9.9 years). If conversion of AF to sinus rhythm did not occur after two hours, the 80 mg sotalol dose was given and later on with four hour intervals 80 mg sotalol were given not exceeding the total dose of 400 mg. In group II patients (15 male, 6 female; mean age, 5?' ?±9'3 ^^ W^ ^ rate m°re thm W0 beatM" WereRESULTS: Conversion of AF to sinus rhythm occurred in 12 of 19 patients (63.2%) having sotalol and in 19 of 21 Pts (90.5%) having digoksin-quinidine (p<0,03). Electric cardioversion was performed in 3 patients of group I and 1 of group II. The mean delay from first tirai drug to sinus rhythm was 13.7+5.9 hours in group 1 and 5.5±1.9 hours in group II (p<0.0001). Four patients who received sotalol were withdrawn because ofbradycardi and hypotension.Finally, we concluded that digoxin-quinidin had better effect on converting acute atrial fibrilation episodes to synus rhythm and incidence of its adverse effects lower than sotalol.

AMAÇ: Bu çalışma, betabloker etkisine de sahip sınıf III antiaritmik ajan olan sotalolun paroksismal atrial fıbrilasyon ataklarının sinüs ritmine çevrilmesindeki etkinliğini değerlendirmek ve digoksinkinidin etkisi ile karşılaştırmak amacıyla yapılmıştır.GEREÇ VE YÖNTEM: Çalışmaya sotalol ve kinidin tedavisi görmeyen, 18-75 yaş arası, kalp hızı >80/dk, sistolik kan basıncı >120 mmHg olan ve 48 saati aşmayan akut atrial fıbrilasyon atağı ile hastanemize başvuran 40 olgu alındı. Olgular tedavi rejimine göre rastgele iki gruba ayrıldı. Grup l 'deyaş ortalaması 53. 1+9. 9 yıl olan 19 olguya (13 E, 6 K) oral olarak 160 mg sotalol verildi. Sinüs ritmine dönmeyen hastalara 2 saat sonra 80 mg ve daha sonra da 4 'er saat ara ile 80 mg sotalol total doz 400 mg olacak şekilde verildi. Grup H 'de yaş ortalaması 57. / '±9.3 yıl olan 21 olguya (15 E, 6 K) kalp hızı >100/dk ise IV digoksin verilerek kalp hızı 100/dk nın altına indirildi. Daha sonra iki saat ara ile 200 mg kimdin ritm sinüse dönene kadar maksimal üç kez verildi. Her iki grupta da son dozdan 12 saat sonra sinüs ritmine dönmeyen hastalara elektriki kardiyoversiyon uygulandı. BULGULAR: Grup l 'de sotalol alan 19 olgunun 12 'si (%63.2) sinüs ritmine dönerken, grup H'de kinidin tedavisi gören 21 olgunun 19'u (%90.5) takip edilen süre içinde sinüs ritmine döndü (p<0.03). Grup l 'de 3 olguya (%15.8), grup tt'de ise 2 olguya (%9.5) elektriki kardiyoversiyon uygulandı. Medikal tedavinin başlaması ile sinüs ritmine dönüş arasındaki ortalama zaman grup l 'de 13.7±5.9 saat, grup H'de ise 5.5+1.9 saat idi ve istatistiki olarak anlamlı bulundu (p<0.0001). Sotalol tedavisi gören 4 hastada semptomatik bradikardi ve hipotansiyon nedeni ile sotalol tedavisi kesildi.Sonuç olarak, çalışmamızda akut atrial fıbrilasyon ataklarının sinüs ritmine çevrilmesinde digoksinkinidin tedavisinin sotalol tedavisine üstün olduğu ve'yan etki insidansının daha düşük olduğu sonucuna varıldı.
