Is factor V Leiden mutation a risk factor for acute miyocardial infarction in young patients with angiographically normal coronary arteries?

Abstract
AIM: The genetic defect of coagulation factor V known as factor V Leiden produces a resistance to degradation by activated protein C (APC). it is caused by a single point mutation in the factor V gene which substitutes arginine at position 506 with a glutamine. Several mechanisms have been proposed to explain the occurrence of myocardial infarction in young patients with angiographically normal coronary arteries, but the role of factor V Leiden in the occurrence of acute myocardial infarction has not yet been fully investigated in this group. Aim of our study was to investigate whether factor V Leiden is a risk factor for acute myocardial infarction in these patients. MATERIAL AND METHODS: The prevalence of factor V Leiden was investigated in 116 patients under 45 years of age with acute myocardial infarction. These patients divided into two groups according to their coroner arteriography: group I (n=57) with normal coronary arteriography and group II (n=59) with critical obstruction in their coronary arteries. The results were compared with those of a group of 60 age and sex matched control subject (group III). Blood samples f rom the patients and controls were analyzed for factor V Leiden mutation by DNA analysis, using the polymerase chain reaction. RESULTS: There was no significant difference in the prevalence of factor V Leiden between patient and control subject. The prevalence of the factor V Leiden mutation 8.4% in group l, 7% in group 2, and 6.6% in group 3 (p>0.05). CONCLUSION: Factor V Leiden mutation is not a risk factor for acute myocardial infarction in young patients with angiographically normal coronary arteries.

AMAÇ: Faktör V Leiden pıhtılaşma sisteminde yer alan faktör V molekülüne ait bir genetik kusurdur. Burada bir mutasyon sonucu faktör V geni üzerinde 506 pozisyonunda argininin yerini glutamin almış ve sonuç olarak da faktör V molekülü aktive protein C' nin parçalayıcı etkisine karşı dirençli hale gelmiştir. Anjiyografik olarak normal koroner arterlere sahip genç hastalardaki miyokard infarktüsü (Mİ) oluşumunu açıklamak için pek çok mekanizma ortaya atılmış, ancak faktör V Leiden (FVL) 'in bu grup hastada İnfarktüs gelişimindeki rolü yeterince incelenmemiştir. Çalışmamızın amacı FVL' in genç ve normal koroner arterlere sahip Mİ geçirmiş hastalarda bir risk faktörü olup olmadığını incelemekti. GEREÇ VE YÖNTEM: FVL sıklığı MI geçiren 45 yaş altındaki 116 hastada incelendi. Bu hastalar koroner anjiyograjilerine göre iki gruba ayrıldı: Grup I (n=57) normal koroner anjiyografısi olan hastalar; Grup II (n=59) koroner arterlerinde kritik darlığı olan hastalar. Bu hastalara ait sonuçlar 60 olgudan oluşan yaş ve cinsiyet eşleştirilmiş sağlıklı kontrol grubuyla (Grup III) karşılaştırıldı. Hasta ve kontrol gruplarından alınan venöz kan örneklerinden FVL mutasyonu polimeraz zincir reaksiyonu (PCR) yöntemiyle analiz edildi BULGULAR: FVL sıklığı açısından her üç grup arasında fark yoktu. FVL sıklığı Grup I'de % 8.4, Grup H'de % 7, Grup IIFde % 6.6 olarak bulundu (p>0.05). SONUÇ: FVL mutasyonunun normal koroner arterlere sahip 45 yaş altı Mİ için bir risk faktörü olmadığı sonucuna varıldı.
