An alternative treatment method for the mechanical prosthetic valve thrombosis: Use of the recombinant tissue plasminogen activator (r-tPA)

Abstract

One of the possible complications of prosthetic heart valve replacement is thrombosis of the artificial valve due to insufficient anticoagulation. The treatment of this devastating complication is surgical thrombectomy or the replacement of the prosthetic valve, but thrombolytic medication can be given alternatively especially for high risk group instead of the surgical option. Streptokinase is the primarily recommended thrombolytic drug. The precise dosage of the recombinant tissue activator for the theraphy of thrombosis of the artificial valves isn't available in the literature. We would like to present our experience in related to the use of the recombinant tissue activator in a 55-year old patient complicating thrombosis of the artificial valve.

Prostetik kapak replasmanı sonrasında gelişebilecek komplikasyonlardan bir tanesi de yetersiz antikoagülan kullanımına bağlı kapak trombozudur. Hayatı tehdit eden bu tabloda primer tedavi cerrahi trombektomi veya kapak replasmanıdır, ancak cerrahi açıdan yüksek risk taşıyan hastalarda alternatif tedavi olarak trombolitik ilaç verilebilmektedir. Trombolitik ilaçlar arasında ilk tercih olarak streptokinaz önerilmektedir. Literatürlerde rekombinant doku plazminojen aktivatörü (rt-PA) için kapak trombozlarında kullanılma dozu ile ilgili bilgi bulunmamaktadır. Bu yazıda, prostetik kapak trombozu gelişen 55 yaşındaki hastada rt-PA kullanımı ile ilgili tecrübemizi sunmak istiyoruz.
