Angiographic restenosis in ephesos coronary stents

Abstract
Aim: Despite the widespread use of coronary stents in percutaneous interventions due to their short and long term beneficial effects, restenosis, which emerges in late periods has not been completely abolished. New stent designs are being developed to prevent restenosis. In this study, we investigated the angiographic restenosis rates of Ephesos coronary stents and related factors. Materials and Methods: Ninety-six patients (66 male, 69% and 30 female, 31%) to whom 135 Ephesos coronary stents (stent diameter range 2.5-4.0 mm, length range 9-23 mm)were implanted successfully and control coronary angiograms (CAG) were performed (160±60 days later) in our catheter laboratory were included. Conventional coronary risk factors such as diabetes mellitus (DM), hyperlipidemia, cigarette smoking, hypertension (HT), and family history and other clinical and angiographic variables were determined. Minimum lumen diameter (MLD), % diameter stenosis (DS) and reference diameter (RD) of the vessel were calculated during the procedure and control CAG by quantitative coronary analysis (QCA). The stents were divided into two groups according to presence or absence of restenosis. Groups were compared with respect to clinical and angiographic variables. Results: Restenosis was observed in 31 (23%) of 135 implanted stents. Preprocedure mean %DS was higher (p=0.02) whereas MLD was lower (p=0.02) in the stents with restenosis. In addition; mean age (p<0.001) and incidence of HT was lower (p=0.043) in the restenosis group. Mean stent diameter was smaller though did not reach statistical significance in restenosis group (p=0.054). By multivariate analysis; younger age (<50 yrs) (p<0.001) and smaller stent size (<3.0 mm) (p=0.016) were found to be independent predictors of restenosis. Conclusion: Our results suggest that Ephesos coronary stents have acceptable restenosis rates and this is comparable with other new generation stents.

Amaç: Kısa ve uzun dönem sonuçları olumlu yönde etkilemesi nedeniyle günümüz perkütan girişimlerin büyük bir çoğunluğunda koroner stentler uygulanmakla birlikte, uzun dönemde ortaya çıkan restenoz tamamen önlenememiştir. Restenozun önüne geçebilmek için her geçen gün yeni stent tasarımları geliştirilmektedir. Bu çalışmada Ephesos koroner stentlerin anjiyografik restenoz oranları ve bunu etkileyen faktörlerin incelenmesi planlandı. Gereç ve Yöntem: Kateter laboratuarımızda başarılı bir şekilde Ephesos koroner stent yerleştirilen ve kontrol koroner anjiyografileri (KAG) (160±60 gün sonra) yapılan 96 hasta [66 erkek (%69), 30 kadın (%31)] ve 135 stent [çap, 2.5-4.0 mm; boy, 9-23 mm] incelendi. Hastaların diabetes mellitus (DM), hiperlipidemi, sigara içme hikayesi, hipertansiyon (HT), aile hikayesi gibi konvansiyonel risk faktörleri ile diğer klinik ve anjiyografik özellikler belirlendi. Stent yerleştirilmesi sırasında ve kontrol KAG'de kantitatif koroner inceleme (QCA) ile minimum lümen çapı (MLÇ), % çap darlığı (ÇD), referans damar çapı (RDÇ) hesaplandı. Stentler, restenozu olan ve olmayanlar olmak üzere iki guruba ayrıldı. Restenozu olan ve olmayan stentlere ait belirlenen klinik ve anjiyografik değişkenler karşılaştırıldı. Bulgular: Uygulanan 135 stentin 31'inde (%23) restenoz saptandı. Restenoz görülen stentlerde işlem öncesi % ÇD daha fazla (p=0.02), MLÇ daha az (p=0.02) idi. Ayrıca restenoz görülen hastaların ortalama yaşı (p<0.001) ve hipertansiyon sıklığı (p=0.043) daha az bulundu. Restenoz grubunda stent çapı daha küçük olmakla birlikte, bu fark istatistiksel anlama ulaşmadı (p=0.054). Çok değişkenli analizde, bu değişkenlerden sadece genç yaş (<50 yaş) (p<0.001) ve küçük (<3 mm) stent çapı (p=0.016) restenozun bağımsız belirleyicileri olarak saptanmıştır. Sonuç: Sonuçlarımız, Ephesos koroner stentlerin restenoz oranlarının kabul edilebilir ve diğer yeni jenerasyon stentlerle karşılaştırılabilir düzeyde olduğunu düşündürmektedir.
