A cardiac rhytm device implantation in a haemodialysis patient: Which side would you prefer?

Abstract
It is known that the need of cardiac rhythm devices (CRD) is about 30% in long term follow up of haemodialysis patients. There is no obvious recommendation for determining appropriate side for CRD implantation in patients with dialyzing AVF. The most common complication after implantation of CRD is central venous stenosis (CVS) which is usually asymptomatic. However; CVS occurred at the same side of the access is symptomatic at the haemodialysis patients dialyzing via arteriovenous fistula (AVF). In case of patients with determined insertion side for CRD implantation is opposite of AVF, there will be encountered with same problems in the future when requires of a new AVF. Determination of ideal side for CRD implantation for patients who require dialiysis via AVF is crucial. The aim of this presented case is to share our experiences about the selection of the appropriate side of insertion side and management of bleeding which is difficult to control during the operation in a haemodialysis patient who CRD implanted.

Hemodiyaliz hastalarının uzun süreli takiplerinde kardiyak ritim cihazlarına (KRC) olan gereksinimin %30 civarında olduğu bilinmektedir. Arteriyovenöz fistülü (AVF) bulunan hemodiyaliz hastalarında KRC implantasyonu için uygun tarafın belirlenmesi için net bir öneri bulunmamaktadır. KRC implantasyonu sonrası en sık karşılaşılan problem santral venöz stenozdur (SVS) ve çoğunlukla asemptomatik seyretmektedir. Ancak, arteryovenöz fistül (AVF) aracılığıyla hemodiyaliz tedavisi alan hastalarda fistülün bulunduğu tarafta gelişebilecek SVS semptomatik seyredecektir. KRC implantasyonu için seçilecek tarafın fistülün karşı tarafı olması durumunda ise bu hastalarda ileriki dönemde yeni bir fistülün gerekmesi sözkonusu olduğunda benzer problemi yeniden gündeme getirecektir. AVF aracılığla diyalize giren hasta grubunda KRC implantasyonu için ideal tarafın belirlenmesi kolay görünmemektedir. Biz, KRC implantasyonu gerçekleştirdiğimiz bir hemodiyaliz hastasında, uygun tarafın seçimi için önerilerimizi ve işlem esnasında kontrolü güç olan kanama ile ilgili tecrübelerimizi aktarmak istedik.
