Cardiac changes in patients with diabetes mellitus

Abstract

AIM: Abnormally prolonged corrected QT interval (QTc) and dispersions of QT-QTc (QTd-QTcd) intervals are associated with a higher risk of ventricular arrhythmias. Microalbuminuria (MA) and diabetic autonomic neuropathy (DAN) are considered as important predictors of cardiovascular events in diabetic patients and decreased heart rate variability (HRV) measures are thought to be associated with autonomic nervous system imbalance. In this study bedside autonomic function tests, HRV measures, QT intervals, microalbuminuria, change in left ventricular morphology and ventricular rhythm problems were studied and relation between those variables were investigated in type II diabetic patients by comparing those results with the healthy control group prospectively. METHODS: Diabetic group consisted of 82 type-II diabetes mellitus (DM) cases (39 females, 58.1±4.0 years) and 40 healthy control cases (18 female, 57.3±2.8years old, p->0.05). RESULTS: DAN was more common in DM cases (29 versus 0, p<0.0001). Diabetic cases had longer QTc interval (0.42±0.02 versus 0.40±0.02 sec, p=0.04), increased QTd (0.04±0.02 versus 0.03±0.01 sec, p<0.0001), QTcd (0.05±0.02 versus 0.04±0.01, p<0.0001), shorter HRV measures: SDANN (111.3±24.8 versus 156.2±9.0 sec), SDNN (135.6±25.5 versus 187.5±13.8 sec), rMSSD (53.9±11.0 versus 73.7±8.4 sec), PNN50 (10.5±6.0 versus 27.1 ±5.2 %), LF (43.8±13.4 versus 60.7±6.1 nu), HF (18.6±6.7 versus 27.1±3.5 nu) (p<0.0001); higher systolic (124.3±9.9 versus 116.4±10.7 mmHg, p<0.0001) and diastolic blood pressures (75.5±9.7 versus 71.9±6.8, p=0.01), left ventricular mass index (LVMI)(111.4±13.5 versus 101.6±12.5 gr/m2, p<0.0001) and higher lown grade ventricular arrhythmias (1.8±0.7 versus 0.7±0.4, p= 0.001). MA was present in 30 of DM cases and in none of control group (p<0.0001). Ventricular rhythm problems showed a positive relation with DAN,QT intervals, and a negative relation with HRV measures. Blood pressures, LVMI and QT intervals showed positive relation with the presence of MA. Presence of DAN showed a negative relation with the measures of HR V. CONCLUSION: Bed side autonomic function tests, measures of HRV and QT intervals show a relation with eachother and ventricular rhythm problems, also presence of MA in DM may cause increase in blood pressure and LVMI resulting with increase in QT intervals predisposing the patient to high risk of ventricular rhythm problems.

AMAÇ: Uzamış düzeltilmiş QT süresi (QTc) ve QT-QTc dispersiyonları (QTd-QTcd) artmış ventriküler aritmi riski ile ilişki gösterir. Diabetes mellituslu (DM) olgularda mikroalbüminüri (MA), diabetik otonomik nöropati (DN) ve düşük kalp hızı değişkenliği (KHD) değerleri kardiyovasküler olaylar için risk oluşturur ve otonomik dengede bir bozukluğu gösterir.Bu çalışmada tip-2 DM olgularında yatak başı otonom fonksiyon testleri, KHD, QT süreleri, MA, sol ventrikül morfolojisi ve ventriküler ritm problemleri prospektif ve kontrollü olarak değerlendirilerek birbirleri ile ilişkileri araştırıldı. YÖNTEM: Gruplarımız 82 tip-2 DM (39 kadın, 58.l±4.0 yaş) ve 40 sağlıklı kontrol (18 kadın, 57.3±2.8 yaş, p>0.05) olgusundan oluşuyordu. BULGULAR: DM grubunda DAN 29 olguda görüldü, kontrol grubunda ise otonom nöropati saptanmadı (p<0.0001). DM grubunda QTc uzun (0.42±0.02'e 0.40±0.02 sn, p=0.04), QTd (0.04±0.02'e 0.03±0.01 sn), QTcd (0.05±0.02'e 0.04±0.01 sn) (p<0.0001) artmıştı, KHD değerleri; SDANN (111.3±24.8'e 156.2±9.0 sn), SDNN (135.6±25.5'a 187.5±13.8 sn), rMSSD (53.9±11.0'a 73.7±8.4 sn), PNN50 (10.5±6.0'a 27.1±5.2 %), LF (43.8±13.4'e 60.7± 6.1 nu), HF (18.6±6.7'e 27.1±3.5 nu) (p<0.0001) düşük; sistolik (124.3±9.9'a 116.4±10. 7 mmHg, p<0.0001) ve diastolik (75.5±9.7'e 71.9±6.8 mmHg, p=0.01) kan basınçları yüksek, sol ventrikül kitle indeksi (SVKİ) (111.4±13.5'a 101.6±12.5 gr/m2, p<0.0001) ve ventrikül aritmilerinin lown sınıflamasına göre değeri (1.8±0.7'e 0.7±0.4, p=0.001) yüksekti. DM grubunun 30'unda MA pozitifken kontrol grubunda görülmedi (p<0.0001). Ventriküler ritm problemleri DN pozitifliği, QT süreleri ile pozitif, KHD ölçümleri ile negatif ilişki gösteriyordu. Kan basınçları SVKİ, QT süreleri ve MA pozitifliği ile pozitif ilişki gösteriyor,DN pozitifliği KHD ölçümleri ile negatif ilişki gösteriyordu. SONUÇ: Yatak başı otonom fonksiyon testleri, KHD ölçümleri ve QT sürelerinin birbirleri ile ve ventriküler ritm problemleri ile belirgin ilişki gösterdiği, aynı zamanda DM'da MA pozitifliğinin bu olgularda ventriküler ritm problemleri riskini artıran kan basıncı ve SVKİ artışına neden olabileceği düşünüldü.
