The effects on restenosis of intravascular ultrasound versus angiography-guided stent implantation

Abstract
AIM: Intravascular ultrasound (IVUS) provides more precise information than angiography about vascular dimensions, and therefore it is used to optimize intracoronary stent implantation. Little is known the effects on restenosis of intravascular ultrasound versus angiography-guided stent implantation. We aimed to investigate the impact of intravascular ultrasound-guided stent implantation on the 6-month restenosis rate. MATERIAL AND METHODS: All patients with successful coronary stent placement were eligible for this study unless they had a major adverse cardiac event during the 1st 30 days after the procedure, and who had a 6-month angiographic and IVUS follow-up study. The 6-month angiographic restenosis rate was evaluated in 113 stents in 104 patients with ischemic heart disease who were randomly allocated to angioplasty and single stent deployment per lesion, with (Group I: 58 patients; 62 stents) versus without (Group II: 46 patients;51 stents) IVUS guidance. Each group had similar clinic and angiographic characteristics. Stent deployment was performed with balloon inflation pressure >12 atm and angiographic residüel stenosis was <10% in both groups. Group I had additional balloon dilation until achievement of IVUS criteria for stent expansion. Over dilation was carried out in 62,9% of stents in Group I. RESULTS: Postprocedure no significant differences were observed in the minimal lumen diameter (MLD) and residüel, stenosis, but the mean referense lumen diameter was significantly larger in Group I (3,18±0,48 mm, 2,96±0,23 mm, p<0,01). At 6 months there was no significant difference in the restenosis rate (22,6 % in Group l vs 33,4% in Group II; (p>0,05). But, MLD (2,33±0,93 mm, l,85±0,84mm; p<0,01) and the mean lumen cross-sectional area-CSA (6,48±2,40mm2 vs 4,83±l,94mm2, p<0,001) were larger in the IVUS- guided group. The mean and minimum stent CSA's (7,62±1,28 mm2, 6,35±l,45mm2; p<0,001 and 7,11±1,83 mm2, 5,88±1,48 mm2; p<0,001) were also higher in Group I. The burden of instent intimal hyperplasia was not correlated with the stent size. The post-stent angiographic measurements were not found as predictors of restenosis, but minimum stent diameter, minimum and mean stent CSA's were the predictors of restenosis by lineer regression analysis. CONCLUSION: Despite insignificant decrease in restenosis rate, IVUS guidance achieved a larger MLD in comparison with angiography-guided stenting. The significant increase in late term lumen size indicate that IVUS guidance in stent deployment may be beneficial.

AMAÇ: Koroner anjiografiye göre damar boyutları hakkında daha kesin bilgi veren İntravasküler ultrason (İVUS) optimum intrakoroner stent implantasyonu amacıyla kullanılmaktadır. İVUS veya anjiografi rehberliğinde stent uygulamalarının restenoz üzerine etkileri konusunda bilgiler sınırlıdır. Bu çalışmada İVUS eşliğinde stent uygulamasının 6.ayda restenoz üzerine etkisi araştırılmıştır. GEREÇ VE YÖNTEM: Çalışmaya başarılı stent implantasyonu yapılmış ve işlem sonrası ilk bir ayda major kardiyak olay geçirmemiş ve 6. ayda anjiografi ve İVUS ile takibi yapılmış hastalar alındı. Altıncı ayda anjiografik restenoz oranı randomize olarak İVUS esliğinde (Grup 1: 58 hasta;62 stent) veya anjiografi rehberliğinde (Grup II:46 hasta;51 stent) anjioplasti ve stent implantasyonu (stent/lezyon=l) yapılan 104 iskemik kalp hastasında 113 stent için değerlendirildi. Grupların klinik ve anjiografik özellikleri benzerdi. Stent uygulamaları 12 atm üzerinde balon inflasyon basınçlarıyla yapıldı ve her iki gruptada anjiografik rezidüel stenoz %10'un altında idi. Grup I' de stent ekspansiyonu için İVUS kriterleri sağlanana kadar ilave dilatasyonlar yapıldı ve ileri dilatasyon stentlerin %62,9 'unda gerekli oldu. BULGULAR: İşlem sonrası anjiografik minimum lümen çapı (MLÇ) ve rezidü darlık yönünden farklılık saptanmadı, fakat ortalama referans lümen çapı Grup I' de daha fazla idi (3,18±0,48 mm, 2,96±0,23 mm, p 0,05). Fakat MLÇ (2,33±0,93 mm, l,85±0,84mm; p<0,01) ve ortalama lümen kesit alanı (6,48±2,40mm2 vs 4,83±l,94mm2, p<0,001) İVUS grubunda daha büyüktü. Ortalama ve minimum stent kesit alanları (7,62±1,28 mm2, 6,35±l,45mm2; p<0,001 ve 7,11±1,83 mm2, 5,88±1,48 mm2; p<0,00l) da Grup I' de fazla idi. Stent içi intimal hiperplazi yükü ile stent kesit alanı arasında korelasyon saptanmadı. Lineer regresyon analizinde stent implantasyonu sonrası anjiografik ölçümler restenozu belirlemede anlamlı bulunmadı. Fakat minimum stent çapı, minimum ve ortalama stent kesit alanları restenoz belirleyicileri idi. SONUÇ: Restenozda önemsiz azalmaya rağmen, İVUS eşliğinde yapılan stent implantasyonlarında minimum lümen çapı ve lümen kesit alanı anjiografi rehberliğinde stent uygulananlara göre daha büyük bulundu. Geç dönemde lümen genişliğindeki artış İVUS' un stent uygulamasında yararlı olabileceğini düşündürmektedir.
