Succesfull treatment of free-floating right heart thrombus and pulmonary embolism with tissue plasminogen activator (tPA): Case report

Abstract
Free-floating right heart thrombi is an uncommon finding in patients with pulmonary embolism. However diagnosis requires urgent treatment. The classical modality of treatment is surgical embolectomy with right heart exploration under cardiopulmonary bypass. Thrombolytic treatment is a faster, readily available and an effective alternative to surgery . In patients with contraindications to surgery or lytic therapy, interventional techniques with basket device or intravenous heparin therapy may be proposed. We thought that presentation of this case of pulmonary embolism with free-floating right heart thrombi who was successfully treated with tPA would be interesting.
Özet
Pulmoner emboli tanısı alan hastalarda sağ kalpte hareketli trombus saptanması nadir bir ekokardiyografık bulgudur. Bu durum acil tedavi gerektirmektedir. Klasik tedavi kardiyopulmoner bypas altında cerrahi embolektomi ve sağ kalbin eksplorasyonudur. Trombolitik tedavi cerrahiye göre daha hızlı, kolay uygulanabilir ve etkili bir alternatif tedavi yöntemidir. Cerrahi ya da trombolitik tedavinin kontrendike olduğu hastalarda kateter eşliğinde girişimsel tedavi ya da intravenöz heparin tedavisi tercih edilebilir. Doku plazminojen aktivatörü (tPA) ile başarılı olarak tedavi edilen , pulmoner emboli ile birlikte hareketli sağ kalp trombusu olan olgumuz sunulmaktadır.
