Spect-myocardial perfusion scintigraphy in the assessment of post stenting coronary

Abstract

Aim: The aims of this study were to evaluate the value of single-photon emission computed tomography (SPECT-MPS) in the detection of post-stenting restenosis, to compare it with exercise ECG and to assess the relation between clinical and angiographic parameters and reversible perfusion defect in the stented territory. Material and Methods: Forty nine consecutive patients (mean age 56.4±10.0; 45 men, 4 women) prospectively underwent coronary angiography, exercise ECG and stress Tc-99m sestamibi SPECT-MPS imaging 6±1 months after stent placement. Results: The overall sensitivity, specificity, positive and negative predictive values for exercise ECG and SPECT in detecting restenosis in individual vessels were, respectively, 39% versus 78% (p=0.03), 73% versus 97% (p=0.03), 47% versus 93% (p=0.02) and 65% versus 89% (p=0.05). The sensitivity of SPECT-MPS was found to be 78% in asymptomatic patients, 80% in patients with prior myocardial infarction, 88% and 70% in patients with single and multivessel disease, respectively. All patients with minimal luminal diameter less than 1.10mm in quantitative coronary angiography had reversible perfusion defects in SPECT-MPS. In logistic regression analysis, reversible perfusion defect in the stented territory was associated with in-stent restenosis. Conclusion: SPECT-MPS is accurate method for identifying in-stent restenosis, its sensitivity is particularly high in patients with low minimal luminal diameter and reversible perfusion defect in the stented territory was associated with in-stent restenosis.

Özet

Amaç: İntrakoroner stent uygulaması sonrası ardışık bir hasta grubunda restenoz tanısının konmasında SPECT-miyokard perfüzyon sintigrafisinin (SPECT-MPS) koroner anjiyografi ile uyumunu saptamak, bu yöntemi egzersiz elektrokardiyografisi ile karşılaştırmak ve SPECT-MPS'de saptanan iskemi ile klinik etkileyen parametreleri ortaya çıkarmaktır. Gereç ve Yöntem: İntrakoroner stent uygulanan ardışık 49 hastaya (yaş ortalaması 56.4±10.0, 45 erkek, 4 kadın) işlemden 6±1 ay sonra koroner anjiyografi, egzersiz elektrokardiyografi ve Tc-99m SPECT-MPS yapıldı. Bulgular: Stent içi restenoz saptanmasında SPECT-MPS'nin duyarlılığı, özgüllüğü, pozitif ve negatif kestirim değerleri egzersiz EKG'ye göre anlamlı olarak artmıştı [sırasıyla %78 ile %39 (p=0.03); %97 ile %%73 (p=0.03); %93 ile %47 (p=0.02); %89 ile %65 (p=0.05)]. SPECT-MPS'nin duyarlılığı asemptomatik hastalarda %78, infarktüs sonrası sorumlu lezyona stent uygulanmış hastalarda %80, tek damar hastalığı bulunanlarda %88 ve çok damar hastalarında %70 idi. SPECT-MPS minimal lümen çapı en dar olan hasta grubunun tümünde, egzersiz EKG ise %50'sinde iskemi saptandı (p=0.015). Lojistik regresyon analizinde SPECT-MPS'de stent bölgesinde saptanan iskemi stent içi restenoz ile ilişkili bulundu (p=0.0001). Sonuç: Stent içi restenozun belirlenmesinde SPECT-MPS'nin uygun bir yöntem olduğu, minimal lümen çapı en düşük gruptaki duyarlılığının çok yüksek olduğu ve SPECT-MPS ile stentli bölgede saptanan iskeminin stent içi restenoz ile ilişkili olduğu ortaya çıkmıştır.
