Response of in-vitro platelet activation induced by agonists in stable angina pectoris patients with type 2b familial hyperlipoproteinemia

Abstract
AIM: Dyslipidemia is an independent risk factor for coronary artery disease (CAD) in both males and females. Platelet activation is seen in the progress of CAD. In this study, the effect of type 2b familial hyperlipoproteinemia (type 2b FHLP) (total cholesterol level >200 % mg, LDL cholesterol>130 % mg, HDL cholesterol<45 % mg, triglyceride>150 mg %) on in-vitro platelet aggregation were investigated in patients with CAD. MATERIAL AND METHODS: One hundred and sixteen patients were included into the study after approval of stable angina pectoris by clinical and angiographical findings. Patients were divided into two groups: Group 1 consisted of 64 patients with stable angina pectoris (SAP) (mean age:54±8, male:48, female:16) who have type 2b FHLP and 52 normolipidemic patients with SAP (mean age:56±11, male:37, female:15) made up Group 2. Patients with diabetes mellitus, hypertension, bleeding diathesis, receiving antiaggregant therapy and statin except aspirin were excluded from the study. Platelet rich plasm a (PRP) samples were prepared from the patients whose venous blood taken before breakfast at room temperature. PRP was treated with adenosin diphosphate (ADP, 10mmol/L), collagen (0.6mgm/ml) and epinephrine (20mmol/L). Platelet aggregation slopes were obtained by using of Born's turbidometric method and these slopes. These slopes were recorded on special milimetric papers. Activation time (second) and activation ratios (amplitude %) of primary and secondary aggregation waves were calculated as quantitatively from the slopes. RESULTS: ADP and epinephrine induced ex-vivo platelet aggregation was increased but, collagen induced in-vitro platelet aggregation was decreased non-significantly (p>0.05) in SAP patients with type 2b FHLP when compared with normolipidemic SAP patients. CONCLUSION: Agonist-induced in-vitro platelet aggregation was not changed in SAP patients with type 2b HPLP.

AMAÇ: Dislipidemi, koroner arter hastalığı (KAH) için cinsiyet farkı olmaksızın bağımsız bir risk faktörüdür. Hem dislipidemi varlığında hem de KAH'nın gelişimi sırasında trombositlerin aktivasyonu söz konusudur. Çalışmamızda kararlı angina pektorisli (KAP) olgularda, tip 2b hiperlipoproteinemin (tip 2b FHLP) in-vitro trombosit agregasyon yanıtı üzerine olan etkilerinin araştırılması amaçlandı. GEREÇ VE YÖNTEM: Çalışmaya, KAP kliniği bulunan ve koroner anjiyografi ile KAH tanısı almış toplam 116 seçilmiş hasta alındı. Tip 2b FHLP saptanan 64 KAP'li olgu (ort. yaş:54±8, 48 erkek, 16 kadın) Grup 1'i oluşturdu. Grup 2 ise, normolipidemik 52 KAP olgusundan (ort. yaş:56±11, 37 erkek, 15 kadın) oluşuyordu. Diabetes mellitus, hipertansiyon, aspirin dışı antiagregan kullanımı, kanama diyatezi ve statin kullanımı öyküsü bulunan olgular çalışma dışı bırakıldı. Olgulardan sabah aç karnına ve oda ısısında alınan venöz kan örnekleri santrifüje edilerek trombositten zengin plazma (TZP) örnekleri hazırlandı. TZP daha sonra, in-vitro adenozin difosfat (ADP, 10mmol/L), kollajen (0.6mgm/ml) ve epinefrinle (20 mmol/L) karşılaştırıldı. Born'un turbidometrik yöntemi kullanılarak trombosit agregasyon dalgaları oluşturuldu ve özel milimetrik kağıtlara yazdırıldı. Agregasyon dalgalarının amplitüd (%) ve süreleri (sn) kantitatif olarak hesaplandı. BULGULAR: Normolipidemik SAP'li olgularla karşılaştırıldığında, tip 2b FHKP'li KAP olgularında ADP ve epinefrinle uyarılmış ex-vivo trombosit agregasyon yanıtının arttığı, kollajenle uyarılmış ex-vivo trombosit yanıtının arttığı, ancak değişikliklerin anlamlı olmadığı (p>0.05) saptandı. SONUÇ: Agonistlerle uyarılmış in-vitro trombosit agregasyonun tip 2b FHLP'li KAP olgularında değişmediği görülmüştür.
