Lipoprotein (A) and lipids in patients with rheumatoid arthritis: Serum levels and relationship with inflammation

Abstract
Aim: Although it has not been yet known exactly, changes of lipid profiles, lipoprotein (a) (Lp(a)) and acute phase reactants are associated with early atherosclerosis in rheumatoid arthritis (RA). Higher Lp(a) and lower HDL-cholesterol levels are independent factors at the pathogenesis of atherosclerosis. In recent years, the associations found between Lp(a) levels and atherosclerotic disorders, diabetes, RA and renal diseases suggest that Lp(a) may be involved in autoimmune reactions. In this study, in patients with RA, serum Lp(a) and lipids levels and relationship with inflammation were examined. Material and Methods: In this study, 87 women with RA diagnosed according to ARA criteria (mean age 45.4±9.4) and 50 healthy women (mean age 44±10.7) were included as a control (C) group. In both groups serum Lp (a), total cholesterol (TC), triglyceride (TG), LDL-cholesterol (LDL-C), HDL-cholesterol (HDL-C) and C-reactive protein (CRP) levels were analysed. LDL-C/HDL-C and TC/HDL-C ratios were calculated. Results: In RA and C groups serum Lp (a) levels were 39.2±20.6 mg/dl and 14.8±9.7 mg/dl (p<0.001), serum TC levels were 188.4±41.8 mg/dl and 185.3±19.3 mg/dl (p>0.05), serum TG levels were 124.5±50.1 mg/dl and 94.6±24.9 mg/dl (p<0.01), serum HDL-C levels were 40.0±7.4 mg/dl and 52.8±4.8 mg/dl (p<0.01) and serum LDL-C levels were 123.4±24.6 mg/dl and 113.3±21.1 mg/dl (p>0.05) respectively. LDL-C/HDL-C ratio in RA and C groups was 3.1±1.2 and 2.1±0.7 respectively (p<0.0001). TC/HDL-C ratio in RA and C groups was 4.7±1.5 and 3.5±0.7 respectively (p<0.0001). While serum CRP level showed positive correlation with Lp(a), it showed negative correlation with HDL-C level (r=0.83, p<0.0001 and r=-0.49, p<0.0001, respectively). It was meaningfull that Lp(a) showed negative correlation with serum HDL-C level (r=-0.36, p<0.001). Conclusions: As a result, it is suggested that higher serum Lp(a), lower HDL-C, higher TG level and high ratio of TC/HDL-C might show high risk of atherosclerosis. Inflammation in RA may cause changes in HDL-C and Lp(a) metabolism.

Amaç: Romatoid artrit (RA)'de, henüz tam olarak bilinmemekle beraber, akut faz reaktanlarıyla birlikte bozulmuş lipid profilinin, erken ateroskleroz gelişiminden sorumlu olabileceği düşünülmektedir. Yüksek lipoprotein (a) [Lp(a)] ve düşük HDL-kolesterol, ateroskleroz patogenezinde bağımsız ve önemli birer faktördür. Son yıllarda gösterilen Lp(a)'nın romatoid artrit, renal hastalıklar ve diyabetle birlikteliği, Lp(a)'nın, immünolojik mekanizmaları etkilemiş olabileceğini düşündürmektedir. Bu çalışmada, romatoid artritli hastalarda Lp (a) ve lipidlerin serum düzeyleri ve inflamasyonla ilişkisi (CRP) araştırıldı. Gereç ve Yöntem: Amerikan Romatizma Cemiyetinin (ARA) kriterlerine göre RA tanısı konan 87 kadın (ortalama yaş 45.4±9.4) hasta ve kontrol (K) grubu olarak sağlıklı 50 kadın (ortalama yaş 44±10.7, p>0.05) çalışmaya alındı ve her iki grupta serum Lp (a), total kolesterol (TK), trigliserid (TG), LDL-kolesterol, HDL-kolesterol ve C-reaktif protein (CRP) düzeyleri ölçüldü; LDL/HDL ve TK/HDL oranları hesaplandı. Bulgular: Tüm olgularda RA ve K gruplarında sırası ile plazma Lp (a) düzeyi 39.2±20.6 mg/dl ve 14.9±9.7 mg/dl (p 0.05), TG düzeyi 124.5±50.1 mg/dl. ve 94.6±24.9 mg/dl (p 0.05) idi. LDL/HDL-kolesterol oranı RA grubunda 3.1±1.2 ve K grubunda 2.1±0.7 idi ve anlamlı fark oluşturdu (p<0.001). TK/HDL-kolesterol oranı ise RA grubunda 4.7±1.5 ve K grubunda 3.5±0.7 hesaplandı ve aradaki fark anlamlıydı (p<0.001). CRP, Lp (a) ile pozitif ve HDL-kolesterol ile negatif anlamlı korelasyon gösterdi (sırasıyla r=0.83, p<0.0001 ve r=-0.49, p<0.0001). Lp(a)'nın ayrıca HDL ile anlamlı negatif korelasyon göstermesi dikkat çekiciydi (r=-0.36, p<0.001). Sonuç: RA'li hastalarda Lp(a) yüksekliğinin, düşük HDL düzeylerinin ve artmış TK/HDL oranı ile yükselmiş TG düzeylerinin ateroskleroz riskini artırabileceği ve RA'de inflamasyon varlığının HDL-kolesterol ve Lp(a) metabolizmasını etkileyebileceği söylenebilir.
