The impact of gender on in-hospital and short-term clinical and angiographic outcome after primary coronary stenting in acute myocardial infarction

Abstract
AIM: The aim of this study was to assess the impact of gender on clinical course and in-hospital mortality and short-term follow-up in patients with acute myocardial infarction (AMI) who underwent primary coronary stenting. MATERIAL AND METHODS: Seven hundred and seventy-four consecutive unselected patients with AMI who underwent direct coronary stenting in the same center were studied retrospectively. The pre-hospital time delay was defined as the time interval from the onset of symptoms to admission to the emergency department. Clinical variables included: age, smoking habits, physical activity, history of hypertension, diabetes mellitus, history of coronary artery disease (documented history of angina and/or previous myocardial infarction), and Q-wave AMI. We compared the patient demographics, angiographic and hemodynamic findings, peak values of CK and MB-CK, prevalence of shock on admission, as well as the clinical outcomes of women versus men. RESULTS: Women (n = 162) represented 20.9% of the sample. On average, women were 7 years older than men (p =0.01). The prevalence of coronary heart disease before the current acute event, maximum values of CK and MB-CK, and angiographic and clinical success were similar in both genders. By univariate comparison, women were less likely than men to be smokers (p <0.0001); more likely to have underlying hypertension (p=0.05), diabetes mellitus (p=0.004), anterior infarctions (p=0.08), cardiogenic shock (p=0.01), and hemodynamic instability (p=0.002); and more likely to have received diuretics (p <0.0001). In hospital mortality was 6.2% (n = 10) for women and 2.6% (n = 16) for men (p = 0.02). Clinical outcomes and short-term follow-up results were similar in both groups. CONCLUSIONS: In our experience, women admitted with acute myocardial infarction and treated by direct coronary stenting are, older than men and have a greater prevalence of shock on admission, diabetes, hypertension, and more severe coronary artery disease. The higher mortality rate in women may relate to the late arrival to hospital, the older age and the more frequent complications. This emphasizes the need to educate women about coronary risk and to urge them to seek early medical care.

AMAÇ: Bu çalışmanın amacı primer stent implante edilen akut miyokard infarktüsü (AMİ) hastalarında, klinik seyir, hastane içi ve kısa dönem takip sonuçlarına cinsiyetin etkisini tayin etmektir. GEREÇ VE YÖNTEM: Akut miyokard infarktüsü tanısıyla gelip ayırım yapılmaksızın direkt koroner stent implante edilen 774 olgu retrospektif olarak incelendi. Ağrı başlangıcından acil servise gelinceye kadar geçen süre hastane öncesi ağrı süresi olarak tanımlandı. Yaş, sigara alışkanlığı, fiziksel aktivite, hipertansiyon, diyabet ve koroner arter hastalığı hikayesi ve Q dalgalı Mİ klinik değişkenler olarak kabul edildi. Kadın ve erkekler arasında demografik, anjiyografik, hemodinamik, klinik sonuçlar ile tepe CK, CK-MB değerleri ve şok sıklığını kıyasladık. BULGULAR: Kohortun 162'si (%20.9) kadın olup erkeklerden ortalama 7 yıl daha yaşlı idi (p=0.01). Koroner arter hastalığı hikayesi, CK ve CK-MB düzeyi, anjiyografik ve klinik başarı oranları her iki cinstede benzerdi. Tek değişkenli kıyaslamada erkekler daha fazla sigara içicisi iken kadınlarda hipertansiyon, diyabet, anterior infarkt, kardiyojenik şok, hemodinamik kararsızlık ve diüretik ihtiyacı daha fazla bulundu (sırasıyla p<0.0001, p=0.05, p=0.004, p=0.08, p=0.01, p=0.002, p<0.0001). Hastane içi mortalite kadınlarda %6.2 (n=10) erkeklerde %2.6 (n=16) bulundu (p=0.02). Klinik sonuçlar ve kısa dönem takip sonuçları ise iki grup arasında benzerdi. SONUÇ: Bizim çalışmamızda direk koroner stent implantasyonuyla tedavi edilen AMİ'li kadınlar erkeklerden daha yaşlı olup, hipertansiyon, diyabet, ciddi koroner arter hastalığı, şok ve mortalite sıklığıda kadınlarda fazla idi. Kadınlarda mortalitenin yüksek olması hastaneye geç ulaşılması, yaşlılık ve daha sık komplikasyon görülmesiyle açıklanabilir. Kadınların koroner riskler ve erken tedavinin yaraları hakkında eğitilmesi faydalı olabilir.
