The relationship between obesity and advanced age in the residents of an old age home

Abstract
Aim: Obesity is an independent risk factor for coronary artery disease (CAD). Obesity increases in both genders along with aging. However it was found that the prevalance of obesity decreased in advanced ages, especially over the age 70. The exact reasons behind this decline seen in later in life is not known yet. In general, in the elderly besides the loss of tissue mass other than fat tissue, the fact that obese individuals cannot reach eighth decade due to natural selection is held responsible for this decline. In our study, we examined the relationship between advanced age and obesity in an old age home population and sought whether there is a difference between those under 80 years old and those that are over the age 80 with respect to obesity markers. Materials and Methods: The study included 82 (50 men and 32 women) elderly residents of an old age home in Afyon. The study group was divided into two groups: age >80 and age <80. Body mass index (BMI) was evaluated by diving the mass in kilograms by the square of the height in meters (kg/m2). The waist and hip circumference were measured respectively at the level of umbilicus and the greater trochanters. Results: The mean age for women was 74±15 and for men 75±8 years. The ratio of men with waist circumference over 102 cm was 9.4% in age <80 group, and was 5.6% in age >80 group. The ratio of women with waist circumference over 88 cm was 42.1% in the age <80 group and 7.7% in the age >80 group. The mean BMI values for men and women aged under 80 years were respectively 26.7±7.2 kg/m2 and 29.9±5.4 kg/m2 and for the ones aged 80 and over were respectively 24.9±4.5 kg/m2 and 27.3±4.8 kg/m2. Conclusion: It was found that after the age of 80, the rate of obesity decreased in both genders of the old age home residents. This decrease was most prominent in the female waist circumference.

Amaç: Obezite koroner arter hastalığı (KAH) için bağımsız bir risk faktörüdür. Obezite yaşla beraber her iki cinsiyette de artmaktadır. Ancak ileri yaşlarda özellikle 70'li yaşlardan sonra obezite prevelansının azaldığı saptanmıştır. İleri yaştaki bireylerde obezite prevelansındaki bu azalmanın kesin nedenleri bilinmemektedir. Genellikle bu durumdan yağ dokusu dışındaki doku kaybı artışının yanısıra obez olmayan kişilerin doğal seleksiyon sonucu ileri yaşlara ulaşma olasılığının daha fazla olması sorumlu tutulmaktadır. Çalışmamızda yaşlı bir huzur evi popülasyonunda ileri yaş ve obezite ilişkisini 80 yaş altı ve üstü olmak üzere iki gruba ayırarak karşılaştırdık. Gereç ve Yöntem: Çalışmaya Afyon'da huzurevinde yaşayan 82 (50 erkek ve 32 kadın) yaşlı birey alındı. Çalışma popülasyonu 80 yaş altı (grup 1) ve üstü (grup 2) olmak üzere iki gruba ayrıldı. Vücut kitle indeksi (VKİ) kilonun boyun karesine bölünmesiyle hesaplandı (kg/m2). Bel çevresi umblikus seviyesinde ve kalça çevresi ise trokanter majus seviyesinde ölçüldü. Bulgular: Kadınların ortalama yaşı 74±15, erkeklerinki ise 75±8 idi. Bel çevresi erkekler için eylem düzeyi olarak belirlenmiş 102 cm'nin üzerinde olanların sıklığı, 80 yaşın altında %9.4, üzerinde ise %5.6, kadınlar için 88 cm'nin üzerinde olanların sıklığı, 80 yaşın altında %42.1 üzerinde ise %7.7 olarak saptandı. Erkek ve kadınların 80 yaş öncesi VKİ ortalamaları sırasıyla (26.7±7.2 kg/m2, 29.9±5.4 kg/m2), 80 yaş sonrası sırasıyla (24.9±4.5 kg/m2, 27.3±4.8 kg/m2) olarak belirlendi. Sonuç: Huzur evinde yaşayan bireylerin her iki cinsiyetinde de 80 yaştan sonra obezite değerlerinde azalma saptandı. Bu azalma kadınlarda bel çevresi için en belirgindi.
