Serum lipids in diabetic cases: The effect of previous coronary events on lipid profile

Abstract

Aim: In our study we aimed to figure out that the success rate of primary and secondary prevention by evaluating the serum lipids in diabetic patients. Material and Methods: 315 diabetic patients enrolled the study and divided into two groups by having coronary events and/or coronary interventions or not. The level of serum total cholesterol, LDL-C, HDL-C, trigliseride, and the rate of lipid-lowering drug usage were compared between groups. Results: Age, sex, frequency of hypertension, and smoking were not different between groups. The mean body mass index, the level of total cholesterol, LDL-C, and triglyceride were significantly lower in secondary prevention group compared to primary prevention group. In addition, the frequency of lipid-lowering drug usage was significantly higher in secondary prevention group. The difference of mean HDL-C level between the groups was not meaningful. Although the success rate to achieve the target level of serum LDL-C was significantly higher in secondary prevention group, the achievement of target levels were not statistically different between groups in relatıon to triglyceride and HDL-C. Conclusion: In diabetic cases, the frequency of lipid-lowering therapy was significantly higher in secondary prevention group compared to primary prevention group. The success rate of achieving the target levels of serum lipids was also higher in secondary prevention group but this not reached statistical significance except LDL-C.

Özet

Amaç: Çalışmamızda diyabetik olguların serum lipid profillerini inceleyerek primer ve sekonder korumada başarı düzeyini araştırmayı amaçladık. Gereç ve Yöntem: Çalışmaya 315 diyabetik olgu alındı ve olgular koroner olay ve/veya koroner girişim geçirenler ve geçirmeyenler olmak üzere iki gruba ayrıldı. Her iki grupta olguların serum total kolesterol, LDL-K, HDL-K, trigliserid seviyeleri tespit edildi ve lipid düşürücü ilaç kullanma oranları karşılaştırıldı. Bulgular: Her iki grup arasında yaş, cinsiyet, hipertansiyon ve sigara kullanma sıklığı açısından fark yoktu. Sekonder koruma grubunda primer koruma grubuna göre; ortalama beden kitle indeksi, total kolesterol, LDL-K, trigliserid seviyeleri önemli derecede düşükken, lipid düşürücü ilaç kullanım sıklığı belirgin olarak daha fazlaydı. Ortalama HDL-K seviyesi ise gruplar arasında farklı değildi. Hedef LDL-K değerine ulaşma başarısı sekonder koruma grubunda istatistiksel olarak belirgin şekilde yüksek iken, hedef trigliserid ve HDL-K değerine ulaşma başarısı açısından gruplar arası fark önemli değildi. Sonuç: Diyabetik olgularda lipid düşürücü tedavi kullanımı sekonder koruma grubunda anlamlı şekilde yüksektir. Hedef serum lipid değerlerine ulaşma başarısı sekonder koruma grubunda primer koruma grubuna göre yine daha yüksektir ancak bu yükseklik LDL-K dışında anlamlı düzeyde değildir.
