Assessment of CHA 2 DS 2 VASC and ATRIA Risk Scores in Patients with Non-Valvular Atrial Fibrillation

Abstract
Objective: The stroke risk should be well evaluated in atrial fibrillation patients for the decision of appropriate anticoagulant treatment. We planned this study to assess CHA 2 DS 2 -VASc and ATRIA risk scores in atrial fibrillation patients. Material and Method: Consecutive one hundred atrial fibrillation patients who were evaluated between May 2017 and October 2017 in our outpatient clinics were enrolled to study. The information that was needed to calculate the CHA 2 DS 2 -VASc scores of the patients was questioned in the patient’s story. The eGFR that was calculated, urine protein and the patient history were used to calculate the ATRIE score. The medical treatments and stroke stories of the patients were recorded. Results: The mean age of patients was 70 years, and %52 were female. Hypertension was the most prevalent comorbidity and was noted in 86% of patients. The rate of warfarin use is 46%, while the rate of apixaban usage is 31%. The total cerebrovascular event was recorded 15% of the patients, while 2 of them (1 acute stroke, 1 transient ischemic attack) were occured acutely during the follow up. The mean ATRIA score was 6.5 ± 2.8 and the CHA 2 DS 2 -VASc score was 3.7 ± 1.4. When the risk scores were divided into low, moderate and high risk groups, the ATRIA score found that 85% of the ischemic stroke patients were in the high-risk group, whereas all ischemic stroke patients were detected in the high-risk group with the CHA 2 DS 2 -VASc scoring system. Conclusions: In this patient population the CHA 2 DS 2 -VASc score showed better outcomes than the ATRIA score to determine ischemic stroke. Patients that were classified as low risk with ATRIA risk score had stroke with the rate of 5%. In our study, we also found that the CHA2DS2-VASc score could be better than the ATRIA risk score, as suggested in the guidelines.

Amaç: Atriyal fibrilasyon hastalarında uygun antikoagülan tedavi kararı için inme riskinin iyi belirlenmesi gerekmektedir. Atriyal fibrilas- yon hastalarında CHA 2 DS 2 -VASc ve ATRİA risk skorlarını değerlendirmek için bu çalışmayı planladık. Gereç ve Yöntem: Bu çalışmaya, poliklinik takiplerinde Mayıs 2017 ile Ekim 2017 tarihleri arasında değerlendirilen ardışık 100 atriyal fib- rilasyon hastası alındı. Hastaların CHA 2 DS 2 -VASc skorlarının hesaplamak için gerekli bilgiler hasta hikayesinden elde edildi, ATRİA skoru- nu hesaplamak için hikaye ile birlikte idrarda protein bakıldı, eGFR hesaplandı. Hastaların medikal tedavileri, inme hikayeleri kaydedildi. Bulgular: Çalışmaya alınan hastaların yaş ortalaması 70 idi ve kadın oranı %52 idi. Hipertansiyon en sık görülen risk faktörüdür ve has- taların %86’sında tespit edildi. Varfarin kullanma oranı %46 iken, yeni oral antikoagülanlardan apiksaban kullanma oranı %31’dir. Toplam serebrovasküler olay oranı %15 olarak bulundu, bunlardan 2 tanesi (1 akut inme, 1 geçici iskemik atak) 6 aylık takip süresi içe- risinde tespit edilen akut olaydı. Ortalama ATRİA skoru 6,5±2,8, CHA 2 DS 2 -VASc skoru ise 3,7±1,4’dür. Risk skorları inme geçirme riskine göre düşük-orta-yüksek olarak gruplara ayrıldığında; ATRİA skorlama sistemi %85 oranında iskemik inmelerin yüksek riskli grupta oldu- ğunu tespit ederken, CHA 2 DS 2 -VASc skoru tüm iskemik inme geçiren hastaların yüksek riskli grupta olduğunu göstermiştir. Sonuç: Non-valvüler atriyal fibrilasyon olan bu hasta popülasyonunda CHA 2 DS 2 -VASc skoru iskemik inmeleri belirlemede ATRİA skoruna göre daha iyi sonuçlar göstermiştir. ATRİA risk skoru ile düşük riskli olarak sınıflanan hastaların %5’inde inme görülmüştür. Bizim çalışma- mızda CHA 2 DS 2 -VASc skorunun, şu anda kılavuzlarda da önerildiği gibi, ATRİA risk skorundan daha iyi performans gösterdiğini bulduk.
