Correlation of glaucoma and postoperative results in pseudoexfoliative patients who underwent phacoemulsification with capsular tension ring

Abstract
Purpose: To investigate the correlation with preoperative existence of glaucoma and postoperative results in pseudoexfoliative patients who underwent phacoemulsification with capsular tension ring (CTR). Materials and Methods: Thirty-five pseudoexfoliative eyes of 34 patients that underwent phacoemulsification with CTR implantation were evaluated. Eyes were examined in two groups one consisting of patients with previously diagnosed pseudoexfoliative glaucoma (17 eyes) under treatment and the other group without glaucoma (18 eyes). Morcher type 14 CTRs were implanted in all eyes following capsulorexis. Groups were compared in terms of preoperative features (age, cataract density, visual acuity, IOP), intraoperative complications (zonule dialysis, capsular perforation, vitreous loss), as well as postoperative first-3-month results (visual acuity, IOP, corneal edema, anterior chamber inflammation). Results: No significant difference was found between groups regarding patient age, cataract density, preoperative visual acuity, and IOP, as well as surgical complication rates. Corneal edema and inflammation scores at the first day and first week visits did not differ, either. Visual acuity and IOP courses during the first three months were also similar in two groups, however, a significant IOP rise was found in the glaucomatous group at the first day visit (p=0.036) which did not continue thereafter. Conclusion: Existence of glaucoma was not found to influence the complication rates and results in pseudoexfoliative eyes that underwent phacoemulsificstion with CTR implantation.
Özet
Amaç: Fakoemülsifikasyon cerrahisi sırasında kapsül germe halkası (KGH) uygulanan psödoeksfoliasyonlu olgularda, operasyon öncesinde glokom varlığı ile postoperatif sonuçların ilişkisinin araştırılması. Gereç ve Yöntem: KGH ile fakoemülsifikasyon cerrahisi uygulanmış, yoğun psödoeksfoliasyonlu ve postoperatif olarak en az 3 ay takipli 34 hastanın 35 gözü değerlendirildi. Gözler preoperatif dönemde tedavi altında psödoeksfoliatif glokomu bulunan (17 göz) ve bulunmayan (18 göz) iki grupta incelendi. Tüm gözlerde kapsüloreksis tamamlandıktan sonra kapsül içine Morcher tip 14 KGH implantasyonu uygulandı. Gruplar preoperatif özellikler (yaş, katarakt sertliği, görme keskinliği, göziçi basınç [GİB]), intraoperatif komplikasyonlar (zonül diyalizi, kapsül perforasyonu, vitreus kaybı), ve postoperatif ilk 3 aydaki erken dönem sonuçlar (görme keskinliği, GİB, korneal ödem, ön kamara inflamasyonu) açısından karşılaştırıldı. Bulgular: Gruplar; yaş, katarakt sertliği, preoperatif görme keskinliği ve GİB'ları, ve cerrahi komplikasyonlar açılarından istatistiksel anlamlı bir farklılık göstermedi. Postoperatif 1.gün ve 1. haftaya ait korneal ödem skorları, ön kamara inflamasyonu açısından da gruplar arasında fark izlenmedi. Gruplar arasında postoperatif ilk 3 aylık dönemdeki görme keskinlikleri ve GİB'larının seyri yönünden farklılık bulunmadı. Glokomlu grupta postoperatif ilk gün preoperatif değerlere göre anlamlı bir GİB artışı ortaya çıktığı (p=0.036), ancak bu durumun daha sonraki kontrollerde devam etmediği izlendi. Sonuç: Psödoeksfoliasyonlu olgularda glokom varlığının, fakoemülsifikasyon cerrahisi sırasında KGH implantasyonunun intraoperatif komplikasyonları ve postoperatif sonuçları açısından ek bir sorun oluşturmadığı kanaatine varıldı.
