Short term complications after penetrating keratoplasty

Abstract
Aim: To evaluate penetrating keratoplasty (PK) results and complications. Material and Method: The patients who underwent PK in Dicle University School of Medicine between March 2012 and March 2013 were reviewed retrospectively. The patients' age, sex, diagnosis, best corrected visual acuity with spectacles before and after surgery, ac- companying surgeries with PK, the diameter of recipient bed and donor cornea, suturing tech- nique, follow up time and complications were noted. Results: Sixty four eyes of 26 female and 38 male patients were included in the study. The mean age of the patients was 51.5±19.5 years. Only PK was performed in 44 patients. PK and accompanying surgeries were performed in 20 patients. The mean diameter of recipient bed was 7.10±0.2 mm, and the mean diameter of donor cornea was 7.57±0.2 mm. Running su- ture technique was performed in 33 eyes and separated suture technique was performed in 31 eyes. The mean follow up period was 7.81±4.01 months. The mean BCVA increased from 0.03±0.65 to 0.2±0.17 postoperatively (p<0.001). The most encountered complications were glaucoma in 11 patients (17.1%), loosening of the sutures in 8 patients (12.5%), graft rejec- tion episode in 4 patients (6.2%), persistent epithelial defect in 4 patients (6.2%) and kerati- tis in 2 patients (3.1%). Conclusion: In corneal diseases PK was effective method. For good prognosis, postoper- ative complications should be known and evaluted carefully. Early diagnosis and tretment were important for graft survival.

Amaç: Penetran keratoplasti (PK) sonuçlarını ve PK sonrası gelişen komplikasyonların incelenmesi. Gereç ve Yöntem: Mart 2012-Mart 2013 tarihleri arasında Dicle Üniversitesi Göz Has- talıkları Ana Bilimdalında penetran keratoplasti (PK) yapılan 64 hastanın dosyası retrospek- tif olarak değerlendirildi. Hastaların yaşı, cinsiyeti, PK endikasyonu, ameliyat öncesi ve son- rasında gözlükle düzeltilmiş en iyi görme keskinlikleri (DEİGK), PK ile birlikte uygulanan di- ğer cerrahi işlemler, trepan çapları, sütür tekniği, takip süresi ve karşılaşılan komplikasyonlar kaydedildi. Bulgular: Çalışmaya 64 hastanın 64 gözü dahil edildi. Olguların ortalama yaşı 51,5±19,5 yıl (en az 12- en çok 81) idi. Olguların 26'sı kadın (%40,6) ve 38'i (%59,4) erkekti. Kırk dört olguda yalnız PK uygulanırken, 20 olguda PK ile birlikte ek cerrahi yapıldı. Kullanılan orta- lama vakum ve punch trepan çapları sırasıyla 7,10±0,2 ve 7,57±0,2 mm idi. Cerrahi sırasın- da 31 olguda tek tek sütürasyon, 33 olguda devamlı sütürasyon tekniği uygulandı. Ortalama takip süresi 7,81±4,01 ay (en az 3 ay, en fazla 16 ay) idi. Olgularda ortalama DEİGK ameli- yat öncesinde 0,03±0,65 iken ameliyat sonrasında 0,2±0,17 (en az 1mps, en fazla 0,8) idi (p<0,001). En sık karşılaşılan komplikasyonlar 11 (%17,1) olguda glokom, 8 (%12,5) olguda sütür gevşemesi, 4 (%6,2) olguda greft reddi, 4 (%6,2) olguda persistan epitel defekti, 2 (%3,1) olguda keratit idi. Sonuç: Uygun kornea hastalıklarında PK etkili bir yöntemdir ve PK sonrası gelişen komp- likasyonların çok iyi bilinmesi ve değerlendirilmesi, bu komplikasyonun erken tanı ve tedavisi prognoz açısından önemlidir.
