Diabetes and pupillary autonomic neuropathy

Abstract

Diabetes mellitus is a multisystem disorder that lead to autonomie neuropathy. To investigate ocular autonomie nerve function was assesed in 40 diabetic patients with, without retinopathy, background retinopathy and proliferative retinopathy and 30 age-sex matched healthy controls. Ocular autonomie function was evaluated by the measurement of pupil cycle time (PCT) and diameter of pupil in resting. There was significantly differences in PCT between patients background retinopathy and proliferative retinopathy and controls (PO.001), but not between patients with background retinopathy and controls. All diabetic patients showed significant differences in pupil diameter in resting, as compared to controls (P<0.05 for background diabetic retinopathy, P<0.001 for other diabetic patients). Our results imply that PCT and pupil diameter measurements as a simple and non-invasive methods may be useful in the evaluation of ocular autonomie function in diabetics.

Özet

Diabetes mellitus otonom nöropatiye neden olan multisistemik bir hastalıktır. Oküler otonomik fonksiyonu incelemek amacıyla fundus lezyonu olmayan, başlangıç retinopatili ve proliferatif dönem retinopatili olan 40 diabetik olgu ile yaş ve cinsiyet dağılımı uygunluk gösteren 30 kontrol olgusu çalışmaya alınmıştır. Oküler otonom fonksiyonu pupil siklus zamanı (PSZ) ve istirahat halindeki pupil çapı ölçümü ile yapılmıştır. PSZ açısından kontrol grubu fundus lezyonu olmayan grup hariç diğer gruplarla istatistiki fark göstermiştir (P<0,001). Pupil çapı değerlendirilmesinde ise kontrol grubu fundus lezyonu olmayan (P<0.05), başlangıç retinopati ve proliferatif dönem retinopatili grup arasında anlamlı bir fark göstermiştir (P<0.001). Bulgularımız PSZ ve pupil çapı gibi basit ve invaziv olmayan teknikler kullanılmasıyla diabetik hastalarda oküler otonomik fonksiyonun değerlendirilebileceğini göstermektedir.
