Evaluation of the motility in acrylic prosthesis after evisceration

Abstract

PURPOSE: To evaluate the motility of the acrylic prosthesis with silicone sphere implantation and patient satisfaction following evisceration. MATERIAL AND METHODS: Evisceration with silicone sphere implantation was performed in 26 cases and the mean follow-up was 32.6 ± 8.4 months (19-52 months). Acrylic prosthesis was fitted in four to six weeks following surgery. The motility of the prosthesis were graded as: Grade 0 : No movement in the prosthesis; Grade 1 : Poor movement with more than 75% limitation; Grade 2 : Moderate movement between 50% to 75% limitation; Grade 3 : Good movement with less than 50% limitation at least in two cardinal gaze positions. The patient satisfaction was evaluated as good, moderate or poor by the patient him- or herself. Pre- and postoperative Multi-dimensional Body-Self Relations Questionnaire (MBSRQ) tests were also performed. RESULTS: Silicone spheres with sizes changing between 14 and 20 mm were used in all cases. In one case extrusion of the implant occured one months after the surgery due to contraction of the scleral tissue and replaced by a smaller sphere. In the follow-up, no movement was present in two cases (7.7%), Grade 1 was observed in nine cases (34.6%), Grade 2 in eleven cases (42.3%), and Grade 3 motility in four cases (15.4%). Fourteen patients (53.8%) revealed good satisfaction, seven patients (27%) moderate, and five patients (19.2%) poor satisfaction. Postoperative MBSRQ test values were significantly higher compared to preoperative values (p=0.008). CONCLUSION: Satisfactory results in mobilization and patient satisfaction can be achieved with correct size silicone sphere implantation and acrylic prosthesis following classical evisceration procedures.

AMAÇ: Eviserasyon sonrası silikon sfer implante edilen gözlerde akrilik protez motilitesi ve hasta memnuniyetinin değerlendirilmesi. GEREÇ VE YÖNTEM: Silikon sfer implantasyonu ile birlikte eviserasyon uygulanan 26 olgunun 26 gözü çalışma kapsamına alındı. Ortalama izlem süresi 32.6 ± 8.4 ay (19-52 ay) idi. Tüm akrilik protezler postoperatif dört ile altıncı haftalarda uygulandı. Protez motilitesi en az iki kardinal bakış yönündeki hareketliliğe göre: Protezde hiç hareket yok (0), az hareketli (%75'den fazla kısıtlılık) (1+), orta derecede hareketli (%50 ile %75 arasında değişen hareket) (2+), belirgin hareketli protez (%50'den az kısıtlılık) (3+) olarak dört grupta değerlendirildi. Hasta memnuniyeti ise hem subjektif olarak (iyi, orta, düşük), hem de ameliyat öncesi ve sonrası yapılan Çok Boyutlu Beden-Self İlişkileri Ölçeği'nin karşılaştırılması ile değerlendirildi. BULGULAR: Tüm vakalarda 14-20 mm arasında değişen ölçülerde silikon sferler kullanıldı. Bir olguda ameliyattan bir ay sonra sklera dokusundaki kontraksiyona bağlı olarak implant ekstrude oldu ve daha küçük bir sfer ile değiştirildi. İzlemde, (3+) motilite dört olguda (%15.4), (2+) motilite 11 olguda (%42.3), (1+) motilite dokuz olguda (%34.6), ve hareketsiz protez iki olguda (%7.7) saptandı. 14 hasta (%53.8) iyi derecede memnuniyet bildirirken, yedi hasta (%27) orta derecede ve beş hasta da (%19.2) düşük memnuniyet bildirdi. Postoperatif Çok Boyutlu Beden-Self İlişkileri Ölçeği değerleri preoperatif değerlerle karşılaştırıldığında anlamlı derecede yüksek olarak saptandı (p=0.008). SONUÇ: Uygun boyutlarda silikon sfer ve akrilik protez implantasyonu ile klasik eviserasyon cerrahisi sonrasında da tatmin edici motilite ve hasta memnuniyeti sonuçları elde edilebilmektedir
