A different approach to the complications in spinal cord injured patients

Abstract
In this study we'evaluated 111 spinal cord injury pa¬tients, who had inpatient care in İstanbul Physical Medicine and Rehabilitation Center during two years, with respect to their complications, the relationship be¬tween the complications and demographical character¬istics, the impact of these complications to hospitalisation time and hospital costs. There were 77 male, 34 female patients, 20 had tetraplegia, 83 had paraplegia and 8 had cauda equina lesion. Of the 111 cases, only 10 of them were complication free where 100 patients had total 232 complications . Most of the patients had more than one complications; 17 had one complication, 37 had two complications, 31 had three complications, 11 had four complications, 2 had five complications, 1 had seven complications together. Uri¬nary tract complications (40 cases), spasticity (35 cases) and pressure sores (27 cases) were the most common complications in our study group. In conclusion we observed that the number of compli¬cations, not the type of complications was the factor that effected the hospitalisation time and the increase in cost.

İstanbul Fizik Tedavi ve Rehabilitasyon Merkezinde iki yıl içerisinde yatarak tedavi edilen 111 spinal kord lezyonlu olguda saptanan komplikasyonlar, olguların demografik özellikleri ile olan ilişkileri, yatış süresine olan etkileri ve tedavi giderleri irdelendi. 77 erkek, 34 kadından oluşan bu seride olguların 20'si tetraplejik, 83'ü paraplejik ve 8'i kauda ekuina lezyonuydu. 11 olgunun komplikasyonsuz seyrettiği, 100 olguda ise toplam 232 komplikasyonun ortaya çıktığı saptandı. Olguların büyük bir çoğunluğunda birden fazla komplikasyon birlikte olup; 37'sinde 2 komplikasyon, 31'inde 3 komplikasyon, 11'inde 4 komplikasyon, 2'sinde 5 komplikasyon, l'inde 7 komplikasyon birlikteliği saptandı. En sık rastlanılan komplikasyonlar üriner sistem komplikasyonları (40 olgu), spastisite (35 olgu) ve bası ülseriydi (27 olgu). Sonuç olarak spinal kord lezyonlu hastada komplikasyonun türü hastanede kalma süresini etkilememekte iken aynı hastada birden çok komplikasyonun birlikteliği hastanede kalma süresini ve dolayısı ile tedavi giderlerini arttırmaktadır.
