Abstract
PURPOSE: Keratoconus is in the first place with aphakic and pseudophakic bullous keratopaty among indications for keratoplasty in our time. In this study, we evaluated preoperative and postoperative conditions of 117 eyes of 107 patients (61 male and 56 female). Who underwent keratoplasty for keratoconus in Kartal Research Hospital between January 1991 and July 1997. SUBJECT AND METHODS: Of these keratoconus patients 42 (%35.8) were among those with palpebral and conjunctival diseases with allergic origin, 3 (%2.5) with Down's syndrome and 1 (%0.8) with retinitis pigmentosa. In our series indication for operation were corneal scarring following acute corneal hidrops in 6 patients (%5.1), intolerance to contact lens using in 21 patients (%17.9) and failure to improve visual acuity which was under 20/200 maximal correction with either spectacles or contact lenses in 90 patients,(%76.9). RESULT: After the operation we found graft clarity in patients (92.3%) and their visual acuity was 20/200 and better in 104 patients (88.8%). In our series more than 3 diopter postoperative astigmatism was found in 59 patients (50.4%). We hope the success rate of keratoplasty in keratoconus will be better when astigmatism complication will be effectively prevented.

AMAÇ: Günümüzde, keratoplasti endikasyonları içinde keratokonus afakik ve psödofakik büllöz keratopatilerle birinci sırayı paylaşmaktadır. Bu çalışmada, Ocak 1991 ile Temmuz 1997 tarihleri arasında, SB. Kartal Eğitim ve Araştırma Hastanesinde, keratokonus nedeni ile keratoplasti olmuş, 107 hastanın 117 gözü ameliyat öncesi ve sonrası değerlendirilmiştir. GEREÇ VE YÖNTEM: 61 hasta erkek, 56 hasta kadındır. 42 hastada (%35.8) keratokonus ile beraber allerjik kapak ve konjonktiva hastalığı, 3 hastada (%2.5) Down sendromu, 1 hastada (%0.8) retinitis pigmentoza mevcuttur. Hastalarımızın ameliyat olma sebebleri; 6 hastada (%5.1) akut korneal hidrops sonrası korneal skar, 21 hastada (%17.9) kontakt lens kullanımından memnuniyetsizlik, 90 hastada ise (%76.9) gözlük veya kontak lensle görme keskinliğinin 0.1'in üzerine cıkartılamamasıdır. BULGULAR VE SONUÇ: Keratokonus nedeni ile keratoplasti olmuş hastalarda, %88.8 oranında 0.1 ve üzeri görme keskinliği, %92.3 oranında greft saydamlığı tespit edilmiştir. %50.4 oranında görülen operasyon sonrası 3 diopri ve üzerinde astigmatizma komplikasyonuna yönelik önleme ve tedavi yöntemleri geliştirildikçe keratokonusta keratoplastinin başarısı dahada artacaktır.
