Comparison of standard extracapsular cataract extraction and endocapsular phacoemulsification methods: Complications, anatomical and functional results

Abstract

PURPOSE: The visual results, postoperative astigmatism and complications for the cases who had standard extracapsular cataract extraction (ECCE} and those who had cataract extraction by endocapsular phacoemulsification (phaco) method have been evaluated and compared. METHODS: 160 eyes of 124 cases who had standard ECCE and intraocular lens implantation and 250 eyes of 207 cases who had cataract extraction by endocapsular phaco method and intraocular lens implantation from January 1996 to September 1998 were included in the study. The visual acuity, postoperative astigmatism and the complications in the standard ECCE with 10 mm corneo-scleral incision and the phaco which was performed with 4-6 mm scleral tunnel incision or 3.5 mm corneal incision were compared. The results taken from the first 100 and the last 150 phaco cases were studied to find out the characteristics of the transition and learning periods. The average follow-up period in the ECCE group and the phaco groups was 5.6 ± 4.9 months and 4.2 ± 1.1 months, respectively. All the cases were evaluated in the first day, first week, first, second and third month postoperatively. RESULTS: While the best corrected visual acuity for the ECCE group in the third month was 5/10 or higher in 78.75% of the eyes, it was 85% for the first phaco group, 93.33% for the second group. Other complications except the iris damage were lower in the first phaco group compared to the ECCE group and the complications in the second phaco group were lower than the other two groups. In the early and late postoperative periods, mean postoperative astigmatism and cylindric refraction changes higher than 1.5 dioptry in phaco groups were significantly lower than those in the ECCE group. CONCLUSIONS: Small incision cataract surgery with phaco method can yield lower surgical astigmatism, better visual rehabilitation and acceptable complication rates than the standard ECCE surgery, even during the transition period.

AMAÇ: Standard Ekstrakapsüler Katarakt Ekstraksiyonu (EKKE) uygulanan olgular ile endokapsüler fakoemülsifikasyon (fako) yöntemiyle katarakt ekstraksiyonu uygulanan olgularda görsel sonuçların, postoperatif astigmatizmanın ve komplikasyonların prospektif olarak değerlendirilmesi ve karşılaştırılması amaçlanmıştır. YÖNTEM: Ocak 1996- Eylül 1998 tarihleri arasında standard EKKE ve Göz içi lens (GİL) implantasyonu uygulanan 124 olgunun 160 gözü ile endokapsüler fako yöntemiyle katarakt ekstraksiyonu ve GİL implantasyonu uygulanan 207 olgunun 250 gözü çalışma kapsamına alındı. 10 mm'lik korneo-skleral insizyonla gerçekleştirilen standard EKKE ve 4-6 mm genişlikli skleral tünel insizyonu ya da 3.5 mm genişlikli korneal insizyon ile gerçekleştirilen fakoda görme keskinlikleri, cerrahiye bağlı astigmatizma ve komplikasyonlar karşılaştırılmış, ilk 100 ve sonraki 150 fako olgusunda sonuçlar incelenerek geçiş ve öğrenme döneminin özellikleri irdelenmiştir. Takip süresi EKKE olgularında ortalama 5.6 ± 4.9 ay, fako olgularında 4.2 ± 1.1 ay olup, tüm olgular postoperatif birinci gün, birinci hafta, birinci, ikinci ve üçüncü ay kontrollerinde değerlendirildiler. BULGULAR: Üçüncü ayda tashihli görme keskinliği EKKE grubundaki gözlerin %78.75 inde 5/10 ve üzerinde iken bu oran fako 1. grupta %85,2. grupta %93.33 idi. Fako 1. grupta iris hasarı dışında diğer komplikasyonlar EKKE grubuna göre düşük, fako 2. grupta tüm komplikasyonlar diğer iki gruba göre düşük idi. Erken ve geç postoperatif dönemlerde fako gruplarında EKKE grubuna göre silindirik kırma kusuru değerleri ve 1.5 dioptri (D)'nin üzerinde silindirik kırma kusuruna rastlanma oranı belirgin olarak daha düşüktü. SONUÇ: Fako ile yapılan küçük kesili katarakt cerrahisi geçiş döneminde dahi daha düşük cerrahi astigmatizma ve kabul edilebilir komplikasyon oranlarıyla görsel rehabilitasyon açısından standard EKKE cerrahisinden daha iyi sonuçlar ortaya koymaktadır.
