The non-contact air-puff tonometer use in the early postoperative intraocular pressure (lop) monitorization after penetrating keratoplasty

Abstract
PURPOSE: to investigate the success and possible complications of the non-contact air-puff tonometry in the early postoperative IOP monitorization after the penetrating keratoplasty. MATERIAL AND METHODS: 44 eyes of the 44 adult patients, 25 (57%) male and 19 (43%) female with the ages of 14 to 75 years (mean=44.97 ± 18.83), who had had a penetrating keratoplasty in the period of February to July 1998 were included in this study. The non-contact air puff tonometry and the Goldmann applanation tonometry were performed at the end of the 1st, 2nd, 3rd and 4th weeks, and 2nd month of the surgery. Statistical analysis was made by Repated Measures ANOVA test. RESULTS: There was no statistically significant difference between the intraocular pressures obtained by the non-contact air-puff tonometer and the Goldmann applanation tonometer (Repated Measures ANOVA; (f=1.881; p=0.122). While performing the air puff tonometry a jet air leakage into the anterior chamber, that was biomicroscopically demonstrated as air bubble in the anterior chamber just after the measurement, was investigated in 3 cases at the end the first week and 2 cases at the end of the second week and one, at the end of the third week after the surgery. After passing to the posterior chamber, the air bubble in the anterior chamber caused a pupillary block that brought out an intraocular pressure rise and shallowing of the anterior chamber, in one case. CONCLUSION: The non-contact air puff tonometer with rts measures closely correlated to the Goldmann applanation tonometer, is an accurate, safe and easy-to-use method to monitorize the intraocular pressure after the 3rd postoperative week in the early postoperative period of the penetrating keratoplasty.

AMAÇ: Penetran keratoplasti sonrası erken postoperatif dönemdeki göz içi basıncı takibinde dokunmasız hava püskürtmeli tonometrinin başarısının ve olası komplikasyonlarının araştırılması. GEREÇ VE YÖNTEM: Şubat 1998 - Temmuz 1998 tarihleri arasında penetran keratoplasti operasyonu uygulanmış 44 yetişkin hastanın 44 gözü çalışmaya dahil edilmiştir.Yaşları 14-75 yıl (ortalama=44.41 ± 18.83) arasında değişen olguların 25'i erkek (%57), 19'u kadındır (%43). Postoperatif 1,2., 3., 4. haftalar ve 2.ay sonunda dokunmasız hava püskürtmeli tonometri ve Goldmann aplanasyon tonometresi ölçümleri yapılmıştır, istatistiksel değerlendirme 'Repeated Measures ANOVA testi ile yapılmıştır. BULGULAR: Postoperatif dokunmasız hava püskürtmeli tonometri değerleriyle Goldmann aplanasyon tonometresi değerleri arasında istatistiksel olarak anlamlı fark saptanmamıştır (Repeated Measures ANOVA; f= 1.881; p=0.122). Üç olguda 1.haftanın sonunda, 2 olguda 2.haftanın sonunda ve 1 olguda 3.haftanın sonunda yapılan ölçümde dokunmasız hava püskürtmeli tonometri esnasında ön kamaraya hızlı hava kaçması ortaya çıkmış ve ölçümden hemen sonra biomikroskopik olarak ön kamarada hava baloncuğu şeklinde izlenmiştir. Ön kamaraya hava kaçan olgulardan birinde, arka kamaraya geçerek pupiller blok oluşturmuş ve ön kamaranın silikleşerek göz içi basıncının aşın yükselmesine neden olmuştur. SONUÇ: Postoperatif erken dönemde, 3.haftadan sonra, dokunmasız hava püskürtmeli tonometri, Goldmann aplanasyon tonometrisi değerlerine çok yakın değerler veren, güvenli ve uygulaması kolay bir göz içi basıncı takip yöntemi olarak saptanmıştır.
