The effects of the incisions for foldable and PMMA lens implantation on surgically induced astigmatism

Abstract
PURPOSE: To compare the surgically induced astigmatism (SIA) after 5.2 mm corneal incision which is minimum wound length that allows polymethylmetacrylate (PMMA) lens implantation and 3.2 mm scleral incision through which a foldable lens can be implanted. MATERIALS AND METHODS: 24 eyes had 5.2 mm superior clear corneal tunnel incision and 20 eyes had 3.2 mm superior sclerocorneal tunnel incisions for pnacoemulsification cataract surgery. Keratometric measurements were taken preoperatively and repeated at 2 days, 1 week, 2 months, 4 months postoperatively from all eyes. Surgically induced astigmatism and astigmatism axis change were analysed by using vector analysis method and Naeser's polar value method. RESULTS: In 5.2 mm corneal tunnel incision group, mean surgically induced astigmatism at two days, 1 week, 2 months, 4 months was 2.18, 1.80, 1.57, 1.22 diopters respectively and in 3.2 mm sclerocorneal tunnel incision group it was 1.24, 1.04, 0.50, 0.50 diopters respectively. By using vector analysis to compare SIA in two groups there was no statistically significant difference at two days (p=0.11) but statistically significant difference was found at 1 week, 2 months, 4 months (p=0.04, p=0.002, p=0.02 respectively). By using Naeser's polar value method to compare SIA axis change in two groups, statistically significant difference was found at two days (p=0.034) but there was no statistically significant difference at 1 week, 2 months, 4 months postoperatively (p=0.14, p=0.38, p=0.13). By using Naeser's polar value method; against the rule astigmatism was found after 1 week postoperatively in both groups. CONCLUSION: The less amount and early stabilization of SIA after 3.2 mm sclerocorneal incision may enhance the success and satisfaction in phacoemulsification cataract surgery.

AMAÇ: Çalışmamızda polimetilmetakrilat (PMMA) lensin konulabileceği minimum kornealinsizyon boyutu olan 5.2 mm ile katlanabilir intraoküler lensin konulabildiği 3.2 mm skleralinsizyondan sonra gelişen postoperatif cerrahi astigmatizma karşılaştırılmıştır.GEREÇ VE YÖNTEM: 24 göze 5.2 mm superior şeffaf korneal tünel insizyonu ve 20 göze 3.2 mm superior sklerokorneal insizyon ile fakoemülsifikasyon katarakt cerrahisi yapıldı. Ameliyat öncesinde çalışmaya alınan tüm gözlerin keratometrik ölçümleri alındı ve bu ölçümler ameliyat sonrası 2.gün, 1.hafta, 2.ay, 4.ayda tekrarlandı. Cerrahi sonucu gelişen astigmatizma miktarı ve astigmatik aks değişikliği vektör analiz metodu ve Naeser'in polar değer metodu ile değerlendirildi.BULGULAR: 5.2 mm korneal tünel grubunda postoperatif 2.gün, 1. h afta, 2.ay, 4.ayda oluşan ortalama cerrahi astigmatizma sırasıyla 2.18, 1.80, 1.57, 1.22 diyoptri ve 3.2 mm grubunda ise1.24, 1.04, 0.50, 0.50 diyoptri olarak izlenmektedir. Bu sonuçların cerrahi astigmatizma miktarı açısından vektör analizi yöntemiyle karşılaştırılmasında gruplar arası postoperatif 2.günde anlamlı fark bulunamamıştır (p=0.11). Ancak postoperatif 1.hafta, 2.ay, 4. aylarda cerrahi astigmatizma açısından gruplar arası fark istatistiksel olarak anlamlı bulunmuştur (sırasıyla p=0.04, p=0.002, p=0.02). Naeser polar değer analiziyle her iki grup cerrahi astigmatizma aksları karşılaştırıldığında postoperatif 2.günde anlamlı fark bulunurken (p=0.034), postoperatif 1.hafta,2.ay, 4.ayda anlamlı fark saptanmam ıstı r(sırasıyla p=0.14, p=0.38, p=0.13). Naeser polar değer analizinde postoperatif 1.haftadan sonra her iki grupta da kural dışı astigmatizma izlenmektedir.SONUÇ: Sonuç olarak 3.2 mm sklerokorneal insizyonun neden olduğu cerrahi astigmatizma miktarının daha az ve astigmatizmanın erken stabilize olmasının klinik olarak fakoemülsifikasyon cerrahisinde tatmini ve başarıyı artıracağını düşünmekteyiz.
