Pars plana vitrectomy in lens luxation

Abstract
PURPOSE: We evaluated the complications of lens dislocation into the vitreous cavity and the effectiveness of pars plana vitrectomy (PPV) in these cases. MATERIAL AND METHOD: Thirty five cases who underwent PPV for the management of dislocated lens consisted the study group. Posterior lens luxation was spontaneous in 4, traumatic in 17 and occurred during cataract surgery in 14 cases. Lens dislocation had developed during phacoemulsification in 10 and during extracapsuler cataract extraction in 4 of the 14 cases. Following PPV the best corrected visual acuity of 1/10 and better is evaluated as functional success. RESULTS: Preoperative visual acuity of cases with traumatic lens luxation was ranging from p+p+ to 5/10, whereas postoperative visual acuity ranged from 1 meter to 9/10. Following vitrectomy visual acuity improved in 10 (58.8%), did not change in 5 (29.4%) and decreased in one (5.9%) of the cases. We could not evaluate the visual acuity of one (5.9%) patient who was a child. We observed retinal detachment in 2 (11.8%), increased intraocular pressure in 2 (11.8%), ocular hypotony in one (5.9%) and vitreous hemorrhage in one (5.9%) case postoperatively. In cases with lens luxation during cataract surgery, visual acuity was ranging from hand motions to 1.5 meters preoperatively. Postoperative visual acuity ranging from 1 meter to 10/10 Following pars plana vitrectomy visual acuity improved in 7 (50%), did not change in 5 (35.7%), decreased in 2 (14.3%) cases. We observed increased intraocular pressure in 2 (14.3%), retinal detachment in 2 (14.3%) cases postoperatively. In cases with spontaneous lens luxation preoperative visual acuity was ranging from hand motions to 4/10. Postoperative visual acuity of these cases ranging from 1 meter to 7/10. Visual acuity was found to be increased in 2 (50%) and unchanged in 2 (50%) of these cases. We did not observe complication following PPV. CONCLUSION: Following PPV we observed functional success in 29.4%-42.8% of cases with lens luxation into the vitreous due to various reasons. We suggest that this propotion will improve especially with the correct orientation of patients with lens luxation during cataract surgery and careful timing of PPV

AMAÇ: Vitreusa lens luksasyonunda pars plana vitrektominin (PPV) yeri ve bu olgularda görülebilecek komplikasyonlar değerlendirildi. GEREÇ VE YÖNTEM: Vitreusa lens luksasyonu nedeniyle PPV uygulanan 35 olgu çalışma kapsamına alındı. Luksasyon nedeni 17 olguda travma, 14 olguda katarakt cerrahisi idi. Dört olguda ise spontan lens luksasyonu mevcuttu. Katarakt cerrahisi sırasında luksasyon gelişen olguların 10'una fakoemülsifikasyon, 4'üne ekstrakapsüler katarakt ekstraksiyonu uygulanmıştı. PPV sonrası düzeltimiş görme keskinliğinin 1/10 ve üstünde olması fonksiyonel başarı olarak kabul edildi. BULGULAR: Travma nedeniyle lens luksasyonu gelişen olgularda preoperatif görme keskinliği p+p+ ile 5/10 arasında, postoperatif görme keskinliği ise 1 metreden parmak sayma (MPS) ile 9/10 arasında idi. Vitrektomi sonrası görme keskinliği 10 (%58.8) olguda arttı, 5 (%29.4) olguda değişmedi, bir (%5.9) olguda ise azaldı. Bir (%5.9) olgu çocuk olduğu için görme keskinliği değerlendirilemedi. Postoperatif 2 (%11.8) olguda retina dekolmanı, 2 (%11.8) olguda göz içi basıncında artma, bir (%5.9) olguda okuler hipotoni, bir (%5.9) olguda vitreus hemorajisi görüldü. Katarakt cerrahisi sırasında luksasyon gelişen olgularda preoperatif görme keskinliği el hareketleri seviyesi ile 1.5 MPS arasında, postoperatif görme keskinliği 1 MPS ile 10/10 arasında idi. PPV sonrası görme keskinliği 7 (%50) olguda arttı, 5 (%35.7) olguda değişmedi, 2 (%14.3) olguda ise azaldı. PPV sonrası 2 (%14.3) olguda göz içi basıncında artma, 2 (%14.3) olguda retina dekolmanı gelişti. Spontan luksasyon olanlarda ameliyat öncesi gör me keskinliği el hareketleri seviyesi ile 4/10 arasında idi. Postoperatif görme keskinliği ise 1 MPS ile 7/10 arasında değişmekteydi. Görme keskinliği 2 (%50) olguda arttı, 2 (%50) olguda değişmedi. Vitrektomi sonrası bir komplikasyon görülmedi. SONUÇ: Değişik nedenlerle vitreusa lens luksasyonu gelişen olgularda PPV sonrası %29.4-%42.8 oranında fonksiyonel başarı sağlandı. Bu oranın özellikle katarakt cerrahisi sırasında luksasyon gelişen olguların yönlendirilmesi ve PPV zamanlamasında daha dikkatli olunmasıyla artabileceği kanısına varıldı.
