Comparison of antiinflammatory effects and intraocular pressure changes of rimexolone %1 and dexamethasone sodium phosphate %0.1 after cataract surgery

Abstract
PURPOSE: To compare the antiinflammatory efficacy and intraocular pressure (IOP) changes secondary to use of topical rimexolone %1 and dexamethasone sodium phosphate % 0.1 following extracapsular cataract extraction (ECCE) and intraocular lens (IOL) implantation. MATERIAL AND METHODS: Between February and April 2000, of the 100 patients who had ECCE and IOL implantation, 50 were randomized to recieve topical rimexolone % 1 and tobramycin % 0.3, and the remaining 50 to topical dexamethasone sodium phosphate % 0.1 and tobramycin % 0.3 4x1. Pain, ciliary hyperemia, corneal edema, cells and flare in the anterior chamber, and intraocular pressure were assessed on postoperative days 1, 3, 10 and 20. RESULTS: Pain and ciliary hyperemia scores were lower in the dexsamethasone group on postoperative day 3 (p<0.05). In other controls, there were no statistically significant differences between the groups. There were no significant differrences in anterior chamber flare and corneal edema scores at all controls (p>0.05). Anterior chamber cell scores were similar in both groups after the postoperative day 10 (p>0.05). IOP changes on all postoperative days were not statistically significantly different between two groups (p>0.05). CONCLUSION: Topical rimexolone % 1 is as efficacious as dexamethasone sodium phosphate % 0.1 in controlling postoperative inflammmation following cataract surgery. They have a similar effect on IOP levels during early postoperative period.

AMAÇ: Ekstrakapsüler katarakt ekstraksiyonu (EKKE) ve intraoküler lens (İOL) implantasyonundan sonra, topikal rimeksolon % 1 ve topikal deksametazon sodyum fosfat % 0.1 'in antiinflamatuar etkinliğini ve göz içi basıncı (GİB) üzerine etkilerini karşılaştırmak. GEREÇ VE YÖNTEM: Şubat ve Nisan 2000 tarihleri arasında EKKE ve İOL implantasyonu uygulanan senil kataraktlı 100 hastanın yarısına (n=50) randomize olarak topikal rimeksolon % 1 ve tobramisin % 0.3, diğer yarısına (n=50) topikal deksametazon sodyum fosfat % 0.1 ve tobramisin % 0.3 4x1 olarak kullanıldı. Postoperatif 1, 3, 10 ve 20. günlerde ağrı, hiperemi, korneal ödem, ön kamarada hücre, flare ve GİB değişiklikleri değerlendirildi. BULGULAR: Ağrı değerleri ve siliyer hiperemi postoperatif 3. günde deksametazon grubunda düşük bulundu (p 0.05). Ön kamaradaki hücre postoperatif 10. günden sonra iki grup arasında anlamlı bir fark göstermedi (p>0.05). Tüm postoperatif günlerde GİB' ları üzerine etkileri arasındaki fark istatistiksel olarak anlamlı değildi (p>0.05). SONUÇ: Katarakt cerrahisi sonrasında postoperatif inflamasyonu kontrol etmede topikal rimeksolon % 1, topikal deksametazon sodyum fosfat % 0.1 kadar etkilidir. Her iki ilacın GİB düzeyleri üzerine etkisi erken postoperatif dönemde farklılık göstermemektedir.
