Clinical findings and treatment results in congenital double elevatory palsy

Abstract
AIM:To evaluate clinical findings and therapy in congenital double elevator palsy. MATERIAL AND METHOD: Data, related to 17 cases who were followed from January 1996 to May 2000 with the diagnosis of congenital double elevator palsy were gathered retrospectively. RESULTS: Involved eyes were found to be more ametropic(p=0.085), more with the rule astigmatic (p=0.028) in the study group. %70 of the cases were ambliyopic, %82 of the cases were hypotropic and %76 of the cases were ptotic. In cases with the mean vertical deviation (MVD) >25D and positive forced duction, inferior rectus recession and partial (5mm) or full transposition of the horizontal recti or modified Jensen, in cases with the MVD<25D and positive forced duction, inferior rectus recession and superior rectus resection, were the preferred surgical procedures. In cases with the MVD <30D and negative forced duction , partial transposition of the horizontal recti, in cases with the MVD>30D and negative forced duction, full transposition of the horizontal recti, were the first procedure. In cases whose deviations could not be corrected completely with the first procedure, recession of the superior rectus of the normal eye in a second procedure was planned. Correction of the horizontal deviations were also planned in the same surgical sessions. The MVD before the surgery was 26.920(14-60) and it was 1.640(0-10) after the surgery. In 6 of the 13 ptotic cases complete or acceptable resolution of the ptosis occurred after strabismus surgery, in the remaining 7 cases ptoses were corrected with brow suspension or levator resection. CONCLUSION: Besides strabismus , deep ambliyopia, which was found to develop in the coexistence of multiple amblyopiogenic factors (deviation, anisometropia, visual deprivation) was another important problem in the congenital double elevator palsy. Development of high astigmatism in the affected eyes could be explained by the presence of vertical recti dysfunction and ptosis. In the surgical treatment, inferior rectus recession becomes necessary if the forced duction test is positive and one of the following surgical procedures; superior rectus resection, transposition of the horizontal recti or modified Jensen could be combined considering the vertical deviation angle. If the patient had horizontal deviation at the same time then transposition of the horizontal recti could be advantageous since it provides correction of the horizontal and vertical deviation at the same time. Ptosis surgery should be planned after correction of the hypotropia.

AMAÇ: Konjenital çift elevatör felçli (KÇEF) olgularda klinik bulgular ve tedavinin değerlendirilmesi. GEREÇ VE YÖNTEM: Kiniğimizin şaşılık biriminde Ocak 1996-Mayıs 2000 tarihleri arasında KÇEF tanısı alan 17 olgu retrospektif olarak değerlendirildi. BULGULAR: Araştırma grubunda etkilenen gözler daha ametropik (p=0.085) ve önemli derecede kurala uygun astigmatik bulundu (p=0.028). Olguların %70'inde ambliyopi, %82 sinde hipotropya %76'sında pitoz vardı. Zorlu düksiyon (ZD) (+) ve vertikal kayma açısı (VKA) >25D ise alt rektusa (AR) geriletme ile horizontal adelelerin tam veya 5mm yukarı transpozisyonu veya modifiye Jensen kombinasyonu, VKA 30D ise horizontal adelelere tam transpozisyon ilk girişim oldu. VKA tam düzelmeyen olgularda normal göz SR'a geriletme planlandı. Aynı zamanda horizontal kayması olan olgularda ise horizontal adelelere geriletme rezeksiyon aynı seansta yapıldı. Cerrahi öncesi ortalama vertikal kayma 26.92D (14-60D) iken cerrahi sonrası bu değer 1.64D (0-10D) olarak elde edildi. Preop.pitozu olan 13 olgunun 6'sında VKA'nın düzelmesi ile pitozda da estetik o larak kabul edilebilir düzelme oldu. 7 olguda ise levator rezeksiyonu veya askı cerrahisi uygulandı. SONUÇ: KÇEF'li olgularda kaymanın yanında derin ambliyopi de önemli bir problem olup, ambliyopiojenik birçok faktörün (kayma, anisometropi ve vizüel deprivasyon) bu olgularda sıklıkla birlikte bulunması sonucu gelişmektedir. Etkilenen gözlerdeki yüksek astigmatizma gelişimi ise vertikal rektuslardaki fonksiyon bozuklukları ve pitozis olması ile açıklanmaktadır. Cerrahi tedavide, ZD (+) ise AR geriletmesi gerekli olup VKA'na göre SR'a rezeksiyon, horizontal adelelere tam veya dereceli transpozisyon veya modifiye Jensen uygulanabilir Horizontal kayması olan olgularda aynı seansta düzeltilebilme imkanı verdiği için transpozisyon avantajlıdır. Pitoz cerrahisi ise hipotropya tamamen düzeltildikten sonra planlanmalıdır.
