The long-term results of exotropia surgery

Abstract
PURPOSE: To evaluate the long-term results of the patients that were operated with diagnosis of exotropia and the factors such as surgical method that can affect the results. MATERIALS AND METHODS: Retrospectively, deviation measurements of 74 exotropic patients (36 male, 38 female) who were operated for exotropia and followed for 3 years minimally, were analysed. Subjects were grouped according to the postoperative deviation measurements at 1 week, 1 year and 3 year postoperatively and evaluated. RESULTS: The mean age at the time of surgery was 11.3 8.4 years (ranged between 10 months and 32 years). Sixtyone patients underwent unilateral recess-resect surgery on the non-fixating eye and thirteen patients had bilateral rectus recessions. Mean follow-up period was 4.11.2 years (3 -7 years). Success rate was 66% at the end of first year and 51% at the end of third year. There was no significant difference between two surgical procedures in terms of long-term findings. Subjects with slight overcorrection in the early postoperative period was found to have better deviation measurements after 3 year. CONCLUSION: Exotropia cases with slight overcorrection in the early postoperative period seem to have better long-term results.

AMAÇ: Ekzotropya tanısı ile ameliyat edilen hastaların uzun dönem takiplerinde kayma değişiminin ve cerrahi yöntem gibi etkili olabilecek faktörlerin incelenmesi. GEREÇ VE YÖNTEM: Ekzotropya nedeniyle ameliyat edilen ve en az üç yıl süreyle kontrolleri yapılan 74 hastada (36 erkek, 38 kadın) kayma değerleri retrospektif olarak incelendi. Olgular ameliyat sonrası 1. hafta, 1. yıl ve 3. yıl sonlarındaki kayma değerlerine göre gruplara ayrılarak değerlendirildi. BULGULAR: Hastalarımızın ameliyat sırasındaki yaş ortalaması 11.3 8.4 yıl (10 ay-32 yaş) idi. Hastalardan 61'ine kayan gözde dış rektusa geriletme- iç rektusa rezeksiyon, 13'üne her iki dış rektusa geriletme ameliyatı yapıldı. Ameliyat sonrası ortalama takip süresi 4.1 1.2 yıl (3-7 yıl) olarak bulundu. Birinci yıl sonundaki %66'lık başarı oranı 3. yıl sonunda %51 olarak bulundu. Geç dönem bulguları açısından ameliyat şekilleri arasında fark görülmedi. Erken dönemde hafif fazla düzeltmenin olduğu hastalarda 3. yıldaki kayma değerleri diğer gruplardan daha iyi bulundu. SONUÇ: Ameliyat sonrası erken dönemde hafif fazla düzeltmenin yapıldığı ekzotropya olgularının uzun dönem sonuçları daha başarılı olmaktadır.
