A prospective study of complications after trabeculectomy

Abstract
OBJECTIVE: This study was performed to examine the complications that occur after trabeculectomy and to determine the complication rates in different types of glaucoma. MATERIAL AND METHODS: This study includes 115 eyes of 103 patients who underwent trabeculectomy between February 1997 and November 1999. The patients were diagnosed as primary open angle glaucoma, chronic narrow angle glaucoma, acute-subacute primary angle clousure glaucoma, chronic primary angle clousure glaucoma, juvenile glaucoma and secondary glaucoma (secondary to pseudoexfoliation, cataract surgery, uveitis, trauma, previous unsuccessful trabeculectomy). After thorough ophthalmologic examinations, trabeculectomy was performed. During the mean follow-up period of 20.68 ± 6.85 months, early and late postoperative complications were recorded and the variations in different types of glaucomas were investigated by using Chi-square test, paired t-test, student's t-test and one-way analysis of variance test. RESULTS: Early postoperative complications occured in 33 (28.6%) eyes while late complications occured in 48 (41.7%) eyes. As early complications, 21 (18.3%) shallow anterior chamber, 19 (16.5%) hypotony, 15 (13%) hyphaema, 8 (7%) choroidal detachment, 5 (4.3%) uveitis, 4 (3.5%) conjunctival leakage, 3 (2.6%) temporary intraocular pressure (IOP) rise, 1 (0.9%) corneal abscess were recorded. As late complications, 28 (24.3%) IOP rise because of filtration failure, 24 (20.9%) cataract formation, 9 (7.8%) increase in c/d ratio, 2 (1.7%) cystic blebs, 1 (0.9%) endophthalmitis were recorded. According to complication rates, no statistically significant difference was observed among different types of glaucoma (p>0.05). CONCLUSION: The low complication rates and the fact that it can be performed on all types of glaucoma suggest that trabeculectomy is a safe and universally employable operation.

AMAÇ: Bu çalışma, trabekülektomi sonrası görülen komplikasyonların incelenmesi ve değişik glokom tiplerindeki komplikasyon oranlarının saptanması amacıyla yapılmıştır. GEREÇ VE YÖNTEM: Bu çalışma, kliniğimizde Şubat 1997-Kasım 1999 tarihleri arasında trabekülektomi ameliyatı olan 103 hastanın 115 gözünü kapsamaktadır. Hastalar, primer açık açılı glokom, kronik dar açılı glokom, akut-subakut primer açı kapanması glokomu, kronik primer açı kapanması glokomu, sekonder glokom (psödoeksfoliasyona, katarakt cerrahisine, başarısız trabekülektomiye, üveite, travmaya sekonder) ve juvenil glokom tanıları almışlardır. Çalışmaya alınan hastalara, tam bir oftalmolojik muayene sonrası trabekülektomi uygulanmıştır. Ameliyat sonrası ortalama 20.68 ± 6.85 ay takip süresince, erken ve geç komplikasyonlar kaydedilmiş ve glokom tipleri arasındaki farklılıklar, Ki-kare testi, student's t testi, paired t testi, tek yönlü varyans analizi kullanılarak değerlendirilmiştir. BULGULAR: Trabekülektomi sonrası 33 (%28.6) gözde postoperatif erken komplikasyonlar, 48 (%41.7) gözde postoperatif geç komplikasyonlar gelişmiştir. Postoperatif erken komplikasyonlar, 21 (%18.3) gözde ön kamara sığlığı, 19 (%16.5) gözde hipotoni, 15 (%13) gözde hifema, 8 (%7) gözde koroid dekolmanı, 5 (%4.3) gözde üveit, 4 (%3.5) gözde konjonktival defekt nedeniyle sızdırma, 3 (%2.6) gözde geçici göz içi basıncı (GİB) yükselmesi, 1 (%0.9) gözde korneal abse; geç komplikasyonlar ise, 28 (%24.3) gözde filtrasyon yetmezliğine bağlı GİB yükselmesi, 24 (%20.9) gözde katarakt, 9 (%7.8) gözde "cup/disc" (c/d) oranında artma, 2 (%1.7) gözde kistik bleb, 1 (%0.9) gözde endoftalmi olarak kaydedilmiştir. Değişik glokom tipleri arasında, komplikasyon gelişme oranları açısından istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0.05). SONUÇ: Trabekülektomi, komplikasyon oranının az olması ve her glokom tipinde uygulanabilmesi nedeniyle, glokomun cerrahi tedavisinde yaygın olarak uygulanan güvenli bir operasyondur.
