Type VII superior oblique palsy (Canine tooth syndrome): Case report

Abstract
Surgical and clinical outcome of a patient involved in a traffic accident who had a Type VII superior oblique palsy is presented. Orbital tomographies were obtained pre and postoperatively. Orthoptic assessment was by preoperative and postoperative analysis of the fields of diplopia, fields of binocular single vision and Hess charts. Surgically, exploration of the superior oblique tendon was carried out and multiple glass foreign bodies were removed from the superomedial orbita. The traction test improved but not to the normal state. The limitation of elevation and depression in adduction improved. The vertical and tortional diplopia dissappeared fully in primary position and abduction, and mostly in adduction. A small field of binocular single vision was obtained. As fusion was demonstrated and a reasonable field of binocular single vision was obtained with a slight chin depression, a second operation was not considered.

Trafik kazası sonucu Tip VII superior oblik paralizisi gelişmiş bir olgunun preoperatif ve postoperatif klinik bulguları değerlendirildi. Preoperatif ve postoperatif dönemde orbita tomografisi çekildi. Tam bir ortoptik muayenenin yanında ana bakış pozisyonlarında prizm örtme testi, diplopi muayenesi, Hess perdesi, binoküler görme alanı ile olgu takip edildi. Yapılan orbitotomi ile superomedial orbitadan multipl cam parçaları çıkarıldı. Superior oblik adele eksplore edildi ve çevre yapışıklıkları açıldı. Girişim sonrası traksiyon testi pozitifliği azaldı ancak normale dönmedi. Postoperatif dönemde klinik muayene ve Hess perdesinde inferior oblik ve superior oblik bakış yönünde motilite kısıtlılığında minimal düzelme olduğu, primer pozisyon, abdüksiyon ve sıklıkla addüksiyonda vertikal ve torsiyonal diplopinin kaybolduğu saptandı. Küçük bir binoküler tek görme alanı oluştu. Hastada füzyonun ve hafif çene depresyonu ile stereopsisin bulunması nedeniyle ikinci bir cerrrahi girişim uygulanmadı.
