Unilateral anterior transposition of the inferior oblique muscle in asymmetric dissociated vertical deviation

Abstract

Purpose: The aim of this study is to present the results of unilateral anterior transposition of the inferior oblique muscle in patients with dissociated vertical deviation and inferior oblique overaction. Materials and Methods: The study included nine patients that were previously selected due to marked ocular preference as a result of decreased vision in one eye. Plus 3 inferior oblique overaction and +3 or +4 dissociated vertical deviation were detected preoperatively in all cases. Unilateral anterior transposition of the inferior oblique muscle operation was performed in nine eyes of nine patients. None of the patients showed alternation of fixation. Results: Inferior oblique overaction was corrected successfully in eight patients. Only one patient showed mild limitation of elevation in the eye in adduction, midline and abduction. Development of manifest dissociated vertical deviation in the nonoperated eye was not detected in any case. Conclusion: Unilateral anterior transposition of inferior oblique muscle for dissociated vertical deviation can give satisfactory results if the operated eye is not taken up the fixation after this unilateral surgery.

Amaç: Dissosiye vertikal deviasyon ve inferior oblik hiperfonksiyonlu hastalarımızda inferior oblik kasının tek taraflı anterior transpozisyonunun sonuçlarını sunmak. Gereç ve Yöntem: Çalışmamız bir gözlerinde az görmeye bağlı olarak belirgin oküler tercihi olan dokuz olguyu kapsamaktadır. Bütün olgularda preoperatif olarak +3 inferior oblik hiperfonksiyonu ve +3 veya +4 dissosiye vertikal deviasyon saptandı. İnferior oblik kasının tek taraflı anterior transpozisyonu ameliyatı dokuz hastanın dokuz gözü üzerinde yapıldı. Hiç bir hasta alterne fiksasyon göstermedi. Bulgular: Sekiz olguda inferior oblik hiperfonksiyonu başarılı bir şekilde düzeltildi. Yalnızca bir olguda addüksiyonda, orta hatta ve abdüksiyonda gözün elevasyonunda kısıtlılık söz konusuydu. Hiç bir olguda ameliyat edilmeyen gözde manifest dissosiye vertikal deviasyon gelişmedi. Sonuç: Tek taraflı cerrahiden sonra ameliyat edilen göz fiksasyon yapmaz ise dissosiye vertikal deviasyon için inferior oblik kasının tek taraflı anterior transpozisyonu tatminkar sonuçlar verebilir.
