HLA DR expression in glaucoma patients on latanoprost or betaxolol treatment

Abstract
Purpose: We aimed to investigate the effects of latanoprost and betaxolol, the frequent drugs of choice used for glaucoma, on ocular surface inflammation. Material and Method: Twenty two eyes of 11 patients on Latanoprost and 18 eyes of 9 patients on Betaxolol treatment took part in our investigation. The right eyes of twelve patients with only refractive problems made up our control group. Routine ophthalmic examination was carried out. Schirmer I and tear break up time (BUT) tests were performed and qualitative corneal sensitivities of all patients were evaluated by cotton-thread test. Cells collected from the patients' lower nasal bulbar-palpebral conjunctiva by the cytobrush technique were smeared on two separate slides. For immunohistochemical investigation, class II major histocompatibility complex antigen HLA DR (DAKO) staining system was used. The results were compared statistically. Results: In the groups using Latanoprost and Betaxolol the mean schirmer test results were 8.91±5.77mm and 8.61±4.63mm, tear break up time was 5.13±3.41 sec and 3.77±1.86 sec, and no significant difference between the groups was found. Although there was a slight decrease in corneal sensitivity in the groups taking medication the change was not significant. In the latanoprost group the HLA (+) cell ratio was significantly increased but in the dominant and maximum staining there was no significant difference between group I and II. There was no marked correlation between duration of drug use and HLA DR expression. Conclusion: The prostaglandin analogues which are clinically eficacious in the control of ocular tension may cause an increase in HLA DR expression in the ocular surface in acute phase but this increase was not correlated with time and thought to have a higher potential than betaxolol in producing subclinical inflammation.

Amaç: Sıklıkla ilk tercih olarak kullanılan glokom ilaçlarından latanoprost ve betaksololün oküler yüzey inflamasyonundaki etkisini araştırmayı amaçladık. Gereç ve Yöntem: Latanoprost %0.005 kullanan 11 hastanın 22 gözü (grup I) ve betaksolol (%0.25) kullanan 9 hastanın 18 gözü (grup II) çalışma kapsamına alındı. Aynı yaş grubundan oluşan ve refraksiyon problemi nedeniyle polikliniğimize başvuran 12 hastanın sağ gözleri kontrol grubunu oluşturdu. Rutin oküler muayene yapıldı. Tüm hastaların Schirmer I, göz yaşı parçalanma zamanı (GPZ) ölçüldü ve kornea duyarlılıkları pamuk iplik yöntemiyle kalitatif olarak değerlendirildi. Hastaların alt nasal bulbar-palpebral konjonktivalarından Hücre Fırçası tekniği ile toplanan hücreler iki ayrı lama yayıldı. İmmunhistokimyasal inceleme için klas II majör histokompatibilite kompleks antijeni HLA DR (DAKO) ye özgü boyama sistemi kullanıldı. Sonuçlar istatistiksel olarak karşılaştırıldı. Bulgular: Latanoprost ve betaksolol kullanan gruplarda ortalama Schirmer testi 8.91±5.77 mm ve 8.61±4.63 mm, göz yaşı parçalanma zamanı 5.13±3.41sn ve 3.77±1.86sn olarak bulundu ve iki grup arasında anlamlı bir fark saptanmadı. Kornea duyarlılıkları minimal azalmakla beraber değişim anlamlı bulunmadı. Latanoprost grubunda HLA (+) hücre oranı anlamlı olarak artmış olduğu ancak dominant boyanmada ve maksimum boyanmada grup I ile II arasında, anlamlı bir fark olmadığı görüldü. Kullanım süresi ile HLA DR ekspresyonu arasında belirgin korelasyon izlenmedi. Sonuç: Oküler tansiyon kontrolünde etkinliğini klinik olarak gördüğümüz prostaglandin analogları inflamasyonu arttırabilmektedirler. Çalışmamızda Latanoprostun oküler yüzeyde akut dönemde HLA DR ekspresyonunu arttırdığı ancak bu artışın süre ile korele olmadığı ve subklinik bir inflamasyon oluşumunda betaksalole göre daha yüksek potansiyel taşıdığı düşünülmüştür.
