Screening of ocular abnormalities in deaf-mute children

Abstract
Purpose: The determination of ocular abnormalities in deaf-mute children and the comparison of these findings with normal subjects of the similar ages. Materials and Methods: This study consisted of 128 deaf-mute students from a special deaf-mute school and 155 healthy students (control group) from a primary school in Kahramanmaraş, and was carried out in January 2002. All of the subjects underwent a full ocular examination including refraction, amblyopia, strabismus, color vision, and anterior and posterior segment examinations. The first examinations of the subjects were performed in their school. The cases who needed advanced examination were invited to the hospital. For the statistical analysis, chi-square test and Fisher's exact test were used according to type of parameters. Results: Ocular abnormalities were found in 29 subjects (22.6%) in deaf-mute group and 32 subjects (20.6%) in control group. The most frequent ocular pathology was refractive errors, 37 (14.4%) eyes in deaf-mute group and 42 eyes (13.5%) in control group (p=0.324). Eight (6%) of deaf-mute subjects and six (4%) of control subjects had different types of strabismus (p=0.217). Frequency of amblyopia was found as 5% (n=6) in deaf-mutes, and 2% (n=2) in controls (p=0.308). Even the frequency of amblyopia and strabismus was more in deaf-mute group numerically, there was no significant difference between the two groups statistically. Also, a case with Goldenhar syndrome, a case with congenital ptosis, and three cases with retinal degeneration were detected in deaf-mute group. Conclusion: This study showed that there was no statistically significant difference between the deaf-mute and normal subjects in respect to frequency and distribution of ocular abnormalities. On the other hand, frequencies of strabismus, amblyopia and retinopathy were higher in deaf-mute group than control group, numerically. Yet, we believe in the necessity of further work to support these findings.

Amaç: Sağır-dilsiz çocuklarda göz patolojilerini saptamak ve bu bulguları aynı yaş grubundan normal çocuklarla karşılaştırmak. Gereç ve Yöntem: Ocak 2002'de yapılan çalışmaya, Kahramanmaraş il merkezinde bulunan İşitme Engelliler Okulu'nun 128 sağır-dilsiz öğrencisi ve kontrol grubu olarak aynı merkezde bulunan bir ilköğretim okulunun 155 öğrencisi dahil edildi. Olgular refraksiyon kusuru, ambliyopi, şaşılık varlığı ve renk görme yönünden değerlendirildi. Tüm olgularda ön ve arka segment muayeneleri yapıldı. Öğrencilerin ilk muayeneleri bulundukları okulda yapıldı. İleri tetkik ve tedavi gerektiren olgular hastaneye çağrıldı. İstatistiksel incelemede karşılaştırılan parametreye göre ki-kare testi ve Fisher'in tam olasılık testi kullanıldı. Bulgular: Sağır-dilsiz grubunda 29 (%22.6) olguda, kontrol grubunda ise 32 (%20.6) olguda göz patolojisi saptandı. Saptanan patolojiler içinde sağır-dilsiz grubunda 37 (%14.4) göz ve kontrol grubunda 42 (%13.5) göz ile refraksiyon kusurları ilk sırayı aldı (p=0.324). İkinci sıklıkta görülen patoloji şaşılıktı. Sağır-dilsiz olguların sekizinde (%6) değişik tiplerde kayma saptanırken, kontrol grubunda altı (%4) olguda kayma saptandı (p=0.217). Sağır-dilsiz grubunda altı (%5), kontrol grubunda üç (%2) olguda ambliyopi saptandı (p=0.308). Ambliyopi ve şaşılık sıklığı sağır-dilsiz olgu grubunda sayısal olarak fazla olmakla birlikte istatistiksel olarak iki grup arasında anlamlı fark saptanmadı. Sağır-dilsiz grubunda bir olguda Goldenhar sendromu, bir olguda konjenital ptozis ve üç olguda retina dejenerasyonu saptandı. Sonuç: Göz hastalıklarının sıklığı ve dağılımı açısından sağır-dilsiz olgular ile aynı yaş grubundan normal olgular arasında anlamlı bir farklılık olmadığı görülmekle birlikte şaşılık, ambliyopi ve retinopati sıklığı sayısal olarak sağır-dilsiz grubunda daha fazla bulunmuştur. Bu bulguları destekleyecek daha geniş kapsamlı çalışmalara gereksinim vardır.
