Abstract
Purpose: To evaluate findings, therapy and prognosis of4 patients with Vogt-Koyanagi-Harada syndrome. Material and Method: Clinical records of four patients with Vogt-Koyanagi-Harada syndrome were investigated retrospectively and findings of history, physical and ophtalmologie examination, radiologie and laboratory assessment were compared with literature. Results: The study group consisted of 4 female patients, whose ages were between 24-50 (mean 31) years old. All of 4 cases had admitted to the hospital with the complaint of bilateral ocular pain and decreased visual acuity. Anterior uveitis was the initial presentation of the disease in three of patients, while in 1 patient anterior uveitis developed during the course of the disease. Exudative retinal detachments which were the most prominent finding leading to diagnosis in these patients; were observed in 3 patients unilaterally and in 1 patient bilaterally. Macular edema and unilateral papillitis were the other findings accounted at initial examination. Vitiligo, poliozis, tinnitus headache and alopecia were the systemic findings observed during the course of the disease. Systemic corticosteroids were the first choice of medical therapeutics in these patients and we achived to control the inflammation with corticosteriods alone in 2 of the patients, where the remaining required supplement therapeutics like azathiopirin and cyclosporine. Conclusion: Although Vogt-Koyanagi Harada syndrome is a rarely encountered disease in our country, it should be considered in the differential diagnosis of exudative retinal detachment. However, it must be considered that exudative retinal detachment may be absent in the initial presentation and together with careful ophthalmologic and systemic evaluation, regular follow-up of the patients are the most important tools to avoid misdiagnosis in these difficult cases.
Özet
Amaç: Bu çalışmada ülkemizde nadir görülen bir hastalık olan Vogt-Koyanagi-Harada (VKH) sendromlu 4 olgu incelenmiş ve tespit edilen bulgular literatür ile karşılaştırılmıştır. Gereç ve Yöntem: Genel ve oftalmolojik muayene, radyolojik ve laboratuvar çalışmaları sonucunda VKH tanısı konulmuş olan 4 olgu retrospektif olarak incelendi. Bulgular: Dördü de kadın olan olguların yaşları 24-50 arasında değişmekte olup yaş ortalaması 31 idi. Dört olgu da her iki gözde ağrı ve görme azlığı şikayetiyle başvurdu. Üç olguda ilk muayenede, l olguda ise daha sonraki kontrollerde ön üveit tespit edildi. Fundus muayenesinde; l olguda iki taraflı diğer 3 olguda ise tek taraflı seröz retina dekolmanı ilk muayenede veya daha sonraki kontrollerde izlendi. İlk muayenede l olguda iki taraflı maküla ödemi, l olguda ise tek taraflı papillit izlendi. Sistemik sorgulama ve muayene sırasında l olguda vitiligo vepoliozis, l olguda ise tinnitus, baş ağrısı ve alopesi tespit edildi. VKH hastalığında en çok kullanılan ajan olan sistemik steroidler ile 2 olguda enflamasyon kontrol altına alındı ve görmeler tama çıktı. Steroidlerin yetersiz kaldığı diğer 2 olguda ise azatiopirin ve siklosporinin yalnız veya steroid ile kombine kullanılarak enflamasyon kontrol altına alındı. Sonuç: Ülkemizde nadir görülmesine rağmen seröz retina dekolmanı tespit edilen hastalarda ayırıcı tanıda öncelikle düşünülmesi gereken hastalık Vogt Koyanagi Harada hastalığıdır. Fakat kimi zaman seröz retina dekolmanı hastalarda izlenemeyebilmekte veya takibinde ortaya çıkabilmektedir. Bu nedenle eşlik eden diğer oftalmolojik muayene bulgularının da iyi tanınıp sistemik bulgularla birlikte değerlendirilmesi ve hastaların dikkatli izlenmesi ayırıcı tanının doğru yapılmasında yararlı olacaktır
