The effect of ocular magnification factors on the results of optic nerve head topographic analysis

Abstract

Purpose: To determine the effect of the ocular magnification factors (OMF) (refractive error, and keratometry) on the measurements of the optic nerve head topographic parameters obtained by a confocal scanning laser ophthalmoscope, Heidelberg retina tomograph II. Material and Methods: Twelve optic nerve head topographic parameters were calculated both before and after the entrance of the OMF of right eye of each of 172 healthy subjects, aged 20 to 35 years, with no systemic diseases and ocular problems, other than refractive errors, using HRT II. The subjects were divided into three distinct refractive error (RE) groups as hypermetropias (RE > 3 D), Emmetropias (-0.75D < RE < 0.75 D) and the myopias (RE < -3 D). Those subjects with disk areas outside the range of 1.75 - 2 mm2, and older than 35 years old were excluded from the study. Results: The mean age and the mean disc area of the subjects were 24.7±4.8 years and 1.94±0.40 mm2, respectively. Upon entrance of the OMF, statistically significant differences were detected in the measurements of all of the optic nerve head parameters (p<0.01) other than disk area, Cup Area/Disc area ratio and the cup shape measure (p>0.01) both in hypermetropia and the myopia groups. Emmetropic subjects did not render significant differences in topographic measurements in any of the parameters studied (p>0.01). Conclusion: Ocular magnification factors have caused significant differences in volume, depth and the area measurements of the optic nerve head topography in high refractive error subjects. Measurements of some of the optic cup parameters have not been affected by OMF. It seems crucial to take this into consideration for follow-up of those population under risk like glaucoma suspects and the ocular hypertensives.

Amaç: Oküler büyütme faktörlerinin (OBF: kırma kusuru ve keratometri), konfokal tarayıcı laser oftalmoskop (Heidelberg retina tomograph II) cihazı ile elde edilen optik sinir başı topografik parametre ölçüm sonuçlarına etkisinin belirlenmesi. Gereç ve Yöntem: Sistemik hastalıkları ve kırma kusurları dışında göze ait problemi olmayan, yaşları 20-35 arasında değişen sağlıklı 172 olgunun sağ gözlerine ait 12 farklı optik sinir başı topografik parametre ölçümleri, HRT II cihazı ile OBF girilmeden ve girildikten sonra olmak üzere iki kez tekrar edildi. Çalışma grubu; kırma kusurundaki (KK) farklılıklarına göre hipermetrop (>3D), emetrop (-0.75D 0.05), diğer parametrelerin hepsinde de, OBF'nin girilmesi sonrasında miyop ve hipermetrop gruplarda istatistiksel olarak anlamlı ölçüm farklılıkları saptandı (p 0.01). Sonuç: Oküler büyütme faktörleri, yüksek kırma kusurlu bireylerde optik sinir başı topografisinin alan, hacim ve derinlik parametrelerinin ölçümlerinde anlamlı farlılıklar oluşturmaktadır. Optik çukur parametrelerinden bazılarının ölçümleri OBF'nden etkilenmemektedir. Glokom şüpheli ve oküler hipertansiyon gibi risk altındaki olguların klinik izlemlerinde bu durumun göz önüne alınması uygun görünmektedir.
