Accompanying Anterior Segment Findings in Patients with Type 2 Diabetes Mellitus

Abstract

Aim: To evaluate the accompanying periorbital and anterior segment pathologies in addition to posterior segment findings in the cases with Type 2 Diabetes Mellitus (DM) diagnoses. Material and Method: One hundred and forty four cases with Type 2 DM diagnoses were included in the study. The patients were questioned regarding age, gender, duration of diabetes, and presence of hypertension. A detailed ophthalmological examination including best corrected visual acuities, measurement of intraocular pressure, biomicroscopic investigation and fundus examination was performed. Schirmer test was performed. The ophthalmological pathologies determined in the cases were recorded. Results: Sixty-seven point four percent of the cases (n=97) were females, 32,6% of them (n=47) were males and the mean age was 55,83±9,85 (32- 90) year. The most common accompanying anterior segment finding to Type 2 DM was blepharitis (n=125, 86,8%). The other findings: dermatochalasis (n=58, 40,3%), cataract (n=47, 32,6%), pingueculae (n=46, 31,9%), whitening of the eyebrow (n=44, 30,6%), dry eye (n=35, %24,3), blepharochalasis (n=29, 20,1%), conjunctivachalasis (n=22, 15,3%), glaucoma (n=13, 9,0%), whitening of the eyelashes (n=9, 6,3%), pterygium (n=6, 4,2%). Proliferative diabetic retinopathy (PDR) was seen in 6,9% of the cases (n=10), mild non-PDR in 8,3% of them (n=12), intermediate non-PDR in 4,9% of the (n=7) and macular edema in 4,9% of them (n=7). Conclusion: In our study, the rates of pingueculae, pterygium, dry eye, proliferative DR and macular edema were found to be similar to the rates stated in the literature. Dermatochalasis, blepharitis, whitening of the eyebrow and the eyelashes and cataract were found to be higher. Eye findings which may accompany should also be investigated carefully during diabetic retinopathy screening in the diabetic patients.

Amaç: Tip 2 Diabetes Mellitus (DM) tanısı bulunan olgularda arka segment bulguları yanında eşlik eden göz çevresi ve ön segment patolojilerinideğerlendirmektir.Gereç ve Yöntem: Tip 2 DM tanılı 144 olgu çalışmaya dahil edildi. Hastalarda yaş, cinsiyet, diyabet süresi, hipertansiyon varlığı sorgulandı.Düzeltilmiş görme keskinliği, göz içi basınç ölçümü, biyomikroskopik inceleme ve fundus muayenesini içeren detaylı oftalmolojik muayene yapıldı. Kurugöz şüphesi olan olgulara Schirmer testi uygulandı. Olgularda saptanan oftalmolojik patolojiler kaydedildi. Elde edilen veriler istatistiksel olarak karşılaştırıldı. Bulgular: Olguların %67,4'ü (n=97) kadın, %32,6'sı (n=47) erkek olup, yaş ortalaması 55,83±9,85 yıl idi. Tip 2 DM'ye en sık eşlik edenön segment bulgusu blefarit (n=125, %86,8) idi. Eşlik eden diğer bulgular sırasıyla; dermatoşalazis (n=58, %40,3), katarakt (n=47, %32,6), pinguekula (n=46, %31,9), kaşta ağarma (n=44, %30,6), kuru göz (n=35, %24,3), blefaroşalazis (n=29, %20,1), konjunktivaşalazis (n=22, %15,3), glokom(n=13, %9,0), kirpikte ağarma (n=9, %6,3), pterjiyum (n=6, %4,2) olarak belirlendi. Olguların %6,9'unda (n=10) proliferatif diyabetik retinopati(PDR), %8,3'ünde (n=12) hafif non-PDR, %4,9'unda (n=7) orta non-PDR, %4,9'unda (n=7) ise maküla ödemi görüldü.Sonuç: Çalışmamızda pinguekula, pterjiyum, kuru göz, proliferatif DR ve maküler ödem oranları literatürde belirtilen oranlara benzer olarak bulundu. Dermatoşalazis, blefarit, kaş ve kirpik beyazlaması, katarakt görülmesi daha yüksek bulundu. Diyabet hastalarında rutin diyabetik retinopati taraması esnasında eşlik edebilecek diğer göz bulguları açısından da özellikle dikkatli olunmalıdır
