The value of immunoglobulin G avidity test for the serological diagnosis of Brucellosis

Abstract
in order to investigate the value of immünoglobulin G (IgG) avidity test for the serological diagnosis of Brucella infections, a total of 118 patients (74 male, 44 female; mean age: 43±18.7 years) were included into the presentstudy. The patients have been diagnosed with the characteristic clinical findings, $geq$1/160 antibody titers in Standard tube agglutination (STA) test and/or blood culture positivities. Brucella spp. have been grown in blood cultures of 78 patients, and STA test results were found positive (>1/160 titers) in 117 patients. The diagnosis of a patient with 1/80 STA titer was based on the blood culture positivity. By enzyme immunoassay (ELISA), 3 patients (2.5%) were found positive for IgM, 14 (11.9%) were positive for IgG, and 101 (85.6%) were positive for both IgM and IgG. The patients who were found IgG positive have been grouped according to their duration of complaints. Group 1 iricluded 99 patients with the history of brucellosis $leq$6 months, and group 2 included 16 patients with the history of brucellosis >6 months. IgG avidity test was performed by ELISA in 115 IgG positive serum samples, with the denaturation substance (8 M urea). The cut-off value for IgG avidity index (Al) was accepted as 40%, and the avidity maturation period was defined as 6 months. As a result, the rates of patients who had low IgG Al in group 1 and 2 were found as 91.9% and 43.7%, respectively, while these rates were 8.1% and 56.3% for the presence of high IgG Al, respectively. The rate of low Al in group 1 was an expected result, while the rate was more than expected in group 2, indicating that Brucella antibodies with low avidity indices would not be helpful for the diagnosis of a recent infection, while Brucella antibodies with high avidity indices would be useful for the elimination of a recent infection.

Brucella enfeksiyonlarının serolojik tanısında immünoglobulin G (IgG) avidite testinin değerinin araştırılması amacıyla planlanan bu çalışmaya, 74'ü erkek, 44'ü kadın olmak üzere toplam 118 hasta (yaş. ortalaması: 43±18.7 yıl) dahil edilmiştir. Brusellozis tanısı; klinik bulgular ve standart tüp aglütinasyon (STA) titresinin $geq$1/160 olması ve/veya kan kültürü pozitifliği ile konulmuştur. Hastaların 78'inin kan kültüründe Brucella türleri üretilmiş ve 117'sinde STA titreleri $geq$1/160 olarak bulunmuştur. STAtitresi 1/80 olarak belirlenen bir hastanın tanısı kan kültür pozitifliğine göre konulmuştur. Enzim immünoassay (ELISA) yöntemiyle, hastaların 3'ünde (%2.5) IgM, 14'ünde (%11.9) IgG ve 101'inde (%85.6) hem IgM hem IgG pozitifliği saptanmıştır. IgG pozitif 115 hasta brusellozis öykü süresine göre 2 gruba (grup 1: öykü süresi $leq$6 ay olan 99 hasta, grup 2: öykü süresi >6 ay olan 16 hasta) ayrılmışlardır. IgG avidite testi, IgG pozitif hasta serumlarında, denatüran madde olarak 8 M ürenin kullanıldığı ELISA ile çalışılmış ve IgG avidite indeks (Aİ) sınırı %40, avidite indeksi değişim süresi ise 6 ay olarak kabul edilmiştir. Sonuç olarak, düşük IgG Aİ'ne sahip hastaların oranı grup 1 'de %91.9, grup 2'de %43.7 olarak bulunurken, yüksek IgG Aİ'ne sahip hastaların oranı grup 1 ve 2'de sırasıyla %8.1 ve %56.3 olarak belirlenmiştir. Düşük Aİ'ne sahip antikorların grup 1 'de beklenilen oranda, ancak grup 2'de beklenilen orandan yüksek olması, düşük aviditeli antikorların yeni kazanılmış enfeksiyonun tanısında yeterli olmadığını, buna karşın yüksek aviditeli antikor varlığının, yeni kazanılmış enfeksiyonun dışlanmasında yararlı olacağını düşündürmektedir.
