Results of Lateral Rectus Recession Surgery with Adjustable Suture Technique for Large Angle Sensorial Exotropia

Abstract

Objectives: To evaluate the results of recession surgery with adjustable suture technique to the amblyopic eye of large angle sensorial ex- otropia patients. Material and Method: Sensorial exotropia patients who had been measured >50 prism diopters with Krimsky prism test and treated with strabismus surgery with adjustable suture technique to the amblyopic eye, were retrospectively evaluated. The mean deviations of postoperative 12 th and 24 th months were statistically compared with preoperative values. The patients who need reoperations were noted. The correlation be- tween postoperative 12 th and 24 th month deviations were statistically evaluated. Results: Preoperative mean deviations of the 17 patients included in the study were 65,88±12,27 prism diopters (range 50-90 prism diopters) at distance and 65,88±11,75 prism diopters (range 50-90 prism diopters) at near. The mean postoperative deviations at 12 th month were 11,58±8,32 prism diopters (range-6-30 prism diopters) at distance and 10,29±8,45 prism diopters (range -6-35 prism diopters) at near. Postoperative 24 th month mean deviations were 11,58±7,27 prism diopters (range-8-25 prism diopters) at distance and 8,58±6,54 prism diopters (range -8 - 20 prism diopters) at near. There was a statistically significant difference in deviations between the preoperative and postoperative 12 th and 24 th months (Wilcoxon test, p<0,05). Reoperations were performed in two patients. Moderate correlation was found between postoper- atively 12 th and 24 th months devations (Spearman test, distance r=0,55, near r=0,41). Conclusion: For large angle sensory exotropia cases, recession surgery with adjustable suture technique can provide the correction which is planned for.

Amaç: Geniş açılı duyusal ekzotropyalı olgularda, ambliyopik göze, ayarlanabilir sütür tekniği ile uygulanan dış rektus geriletme cerrahi- si sonuçlarını değerlendirmek. Gereç ve Yöntem: Krimsky prizma testi ile 50 prizma diyoptrisi veya üzerinde olan ve ambliyopik göze ayarlanabilir sütür tekniği ile geri- letme cerrahisi uygulanan duyusal ekzotropyalı olgular retrospektif olarak değerlendirildi. Olguların postoperatif 12. ay ve 24. ay uzak ve ya- kın ortalama kayma ölçüm dereceleri, preoperatif değerlerle istatistiksel olarak karşılaştırıldı. Reoperasyon yapılan olgular kaydedildi. Olgulardaki postoperatif 12. ay ve 24. ay arasındaki kayma ölçümleri arasındaki korelasyon istatistiksel olarak değerlendirildi. Bulgular: Çalışmaya alınan 17 olgunun preoperatif ortalama kayma değerleri uzakta 65,88±12,27 prizma diyoptrisi (50-90 prizma diyop- trisi arası) yakında ise 65,88±11,75 prizma diyoptrisi (50-90 prizma diyoptrisi arası) olarak ölçüldü. Postoperatif 12. ayda ortalama uzak ve yakın kaymalar sırasıyla 11,58±8,32 prizma diyoptrisi (-6-30 prizma diyoptrisi arası) ve 10,29±8,45 (-6-35 prizma dioptrisi) arası olarak bu- lundu. Postoperatif 24. ayda ise uzakta 11,58±7,27 prizma diyoptrisi (-8-25 prizma diyoptrisi arası), yakında 8,58±6,54 prizma diyoptrisi (-8- 20 prizma dioptrisi) olarak saptandı. Olgularda preoperatif ölçümler ile postoperatif 12. ay ve 24. ay kayma ölçümleri arasında istatistiksel ola- rak anlamlı fark bulundu (Wilcoxon test, p<0,05). İki olguya reoperasyon uygulandı. Postoperatif 12. ay ve 24. ay kayma değerleri arasında or- ta dereceli pozitif korelasyon bulundu (Spearman test, uzak r=0,55, yakın r=0,41). Sonuç: Geniş açılı duyusal ekzotropya tanılı olgularda, ayarlanabilir sütür tekniği ile geriletme cerrahisi, hedeflenen düzeltme miktarını sağlayabilmektedir.
