Nöroendokrin Tümörlerin SPECT-BT Cihazi İle Tanısal Görüntülemesinde Tc-99M İşaretli Somatostatin Reseptör Ajanı (99mTc-Hynic-Oktreotate) Kullanımının Etkinliğinin Araştırılması

Abstract
Objective: The aim of this study was to assess the diagnostic efficiency of 99mTc-EDDA/HYNIC-Octreotate in comparison with 111Inpentetrotide scintigraphy in the detection of neuroendocrine tumors. This study also evaluates the impact of SPECT-CT hybrid imaging on somatostatin receptor scintigraphy (SRS) interpretation and clinical management of these tumors. Methods: Fourteen patients were included in the study. All patients underwent a whole body and SPECT-CT imaging with both 99mTcEDDA/HYNIC-octreotate and 111In-pentetrotide. Images were evaluated both visually and semiquantitatively. Results: On patient basis, the diagnostic results of both studies were similar. The number of lesions detected by 99mTc- EDDA/HYNICOctreotate were higher than the number of lesions detected by 111In-pentetrotide however the difference was not significant (40/43( 93%), 36/43 (83%) p=0.109). Semiquantitative analysis showed higher tumor/organ count ratios for both whole-body and SPECT 99mTc- EDDA/HYNIC-Octreotate scans. Conclusion: The results of this study suggested that, 99mTc- EDDA/HYNIC-Octreotate may be a better alternative to 111In- pentetrotide due to high image quality and lower radiation dose. SPECT/CT is a valuable tool for the assessment of neuroendocrine tumors by providing the precise anatomic localization of scintigraphic findings thus improving lesion detectability and characterization.
Özet
Amaç: 99mTc-EDDA/HYNIC-TATE, somatostatin reseptör görüntülemede etkinliği araştırılan yeni bir somatostatin analoğudur. Çalışmada amacımız nöroendokrin tümörlerde 99mTc-HYNIC-oktreotate ile somatostatin reseptör görüntülemenin etkinliğini 111Inpentetrotide ile karşılaştırmak ,ayrıca hibrid görüntüleme yöntemi olan SPECT-BTnin nöroendokrin tümörlerin tanısına katkısını araştırmaktır. Metod: Çalışmaya 14 hasta dahil edilmiş, tüm hastalara 99mTc-HYNIC-oktreotate ve 111In-pentetrotide ile SPECT-BT görüntüleme yapılmıştır. 99mTc-HYNIC-oktreotate ve 111In-pentetrotide görüntüleri görsel, sayısal, ve semikantitatif olarak karşılaştırmalı olarak hasta ve lezyon bazında değerlendirilmiştir. Bulgular: Hasta bazında yapılan değerlendirmede 111In-pentetrotide çalışması ile patolojik tutulum saptanan tüm hastalarda 99mTcHYNIC-Oktreotate ile de aynı lokalizasyonda patolojik tutulum başarı ile saptanmıştır. 99mTc-HYNIC-Oktreotate ile saptanan patolojik odak sayısı 111In-pentetrotide ile saptanan odak sayısından fazla olmakla birlikte aradaki fark istatistiksel olarak anlamlı bulunmamıştır ( sırası ile 40/43 (%93), 36/43 (%83), p=0.109) Semikantitatif analizde, planar ve SPECT görüntülerde 99mTc-HYNIC-Octreotate çalışmasındaki tümör/karaciğer, tümör/böbrek ve tümör/zemin aktivite tutulum oranları 111In-pentetrotid çalışmasındaki aynı oranlardan anlamlı düzeyde yüksek bulunmuştur (sırası ile p değerleri 0,015, 0,007, 0,066 ). SPECT-BT çalışması ile yüksek görüntü kalitesinde tutulum alanlarının anatomik lokalizasyonu daha net tanımlanmış, görüntüleme alanına giren fizyolojik tutulumların tümöral tutulumlardan ayırt edilmesi daha doğru yapılabilmiştir. Sonuç: 99mTc-HYNIC-Oktreotate somatostatin reseptör görüntüleme 111In-pentetrotide ile uyumlu sonuçlar vermiştir. 99mTc-HYNIC-oktreotate, yüksek görüntü kalitesi, düşük radyasyon maruziyeti, tek gün görüntüleme protokolü ile 111In-pentetrotide ile somatostatin reseptör görüntülemeye iyi bir alternatif olarak görülmektedir. Ayrıca SPECT-BT görüntüleme ile anatomik lokalizasyon, fizyolojik/tümöral tutulum ayrımı daha net yapılmakta, tüm bu özellikler tanı ve tedaviye doğru katkı şeklinde yansımaktadır.
