Preoperative Forced-Air Warming Combined With Intraoperative Warming Versus Intraoperative Warming Alone in the Prevention of Hypothermia During Intraabdominal Surgery

Abstract
Objective: Preventing hypothermia is important for anestetic management. Forced-air is a very effective method for preventing complications of hypothermia. Material and Methods: After obtaining Etical Commitee approval, this study was conducted as a prospective, randomized study including 70 patients who were planned to undergo elective abdominal surgery between 7 December 2015-7 February 2016 in surgery clinic. Patients between age 16-65, ASA I-III who were planned to undergo elective abdominal surgery were included in the study. Patients were devided into two groups, group 1 (n=35) randomized, not heated before operation, heated during operation by forced-air method and group 2 (n=35) heated before operation in PACU. All patients were heated with forced-air during operation. Results: In the analyzing of two groups, the frequency of postoperative respiratory distress, cardiac morbidity, intraoperatif blood volume loss, duration of neuromuscular blockage was significantly higher in the group 1 (n=35) that were not preheated before the surgery than the group 2 (n=35) that were preheated before the surgery. The average body temperature of group 2 (n=35) after induction, 15., 30., 60., minutes, before extubation and at the PACU was significantly higher than group 1 (n=35) (p:0,001; p<0,05). Statistically there is no significant differences in the ratio of postoperative surgical area infection risk between two groups. Conclusion: Preoperative body temperature must be measured before the patient arrrives at operating room and its follow up is a very important parameter. Patients should be transferred to operating room when their body temperature is at least above 36°C and if the body temperature is below 36°C, patients should be heated by active systems. Patients should be heated for 30 minutes with forced-air systems before surgery at operating room.

Amaç: Hipoterminin önlenmesi için preoperatif ısıtma anestezi yönetiminin önemli komponentlerinden biridir. Forced-air yöntemi hipoterminin bu ciddi komplikasyonlarını önlemede çok etkili bir yöntemdir.Gereç ve Yöntem: Çalışma, Etik Kurul onayı alınarak 7 Aralık 2015-7 Şubat 2016 tarihleri arasında, 70 olgu üzerinde, prospektif, raslantısal ve tek kör olarak planlandı. ASA skoru I-III olan, 18-65 yaş arası, elektif genel cerrahi batın operasyonu planlanan olgular dahil edildi. Hastalar randomize Grup I (n=35); preoperatif ısıtılmadan yalnızca intraoperatif forced-air yöntemiyle ısıtılanlar ve Grup II (n=35) operasyondan önce forced-air yöntemiyle postoperatif derlenme ünitesinde (PACU) 30 dk. ısıtılan ve bu sırada hemodinamik takipleri yapılan hastalar olmak üzere 2 gruba ayrıldı. Tüm hastalar aynı zamanda intraoperatif forced-air yöntemiyle ısıtıldı. Her 2 grupta indüksiyon ajanı olarak propofol 2,5 mg/kg, rokuronyum bromid 0.6-1.2 mg/kg kullanıldı.Bulgular: Her iki grup arasında yapılan karşılaştırmada, Grup I (n=35) hastaların kas gevşetici etkisinin süresi, intraoperatif kan kaybı miktarı (ml), postop solunum sıkıntısı görülme sıklığı, kardiyak morbidite sıklığı, Grup II (n=35) hastalarından istatistiksel olarak anlamlı düzeyde fazla bulunmuştur. Grup II'nin indüksiyon sonrası, 15. dk., 30. dk., 60. dk., ekstübasyon öncesi ve PACU vücut ısısı ortalamaları Grup I'den anlamlı düzeyde yüksektir (p<0,05). Sonuç: Preoperatif hastaların vücut sıcaklıkları hasta ameliyat odasına gelmeden önce ölçülmeli ve ısı takibi dikkat edilmesi gereken bir parametre olmalıdır. Hastalarının vücut sıcaklıklarını en az 36°C olduğunda ameliyat odasına transfer etmeleri, 36°C'nin altındaki hastaları aktif olarak ısıtmaları gerekliliği konusunda bilgilendirilmelidirler. Ameliyat odasına gelen hastalar öncelikle derlenme ünitesinde forced-air yöntemiyle en az 30 dk. ısıtılmalıdır.
