Genetic basis of thyroid carcinoma: A review

Abstract
Thyroid cancer is the most widespread endocrine malignancy and responsible for the majority of en- docrine cancer-related deaths each year. Germ-line transition has been demonstrated in differentia- ted follicular cancers originating from thyroid folli- cular epithelium, similar to medullary thyroid can- cer. Although it is not shown on a detailed study that a mutation or a gene is responsible for it, it is still possible to speak of genetic susceptibility to thyroid cancer. It focuses on a number of possible tumor suppressor genes and oncogenes. The app- lication of molecular technologies on the study of these tumors over the past two decades has shed considerably light on the association of genetic abnormalities with the development of the major thyroid tumor types. Although genetic mutations like RET, TRK, BRAF and RAS are found in approximately 70% of PTCs, they rarely overlap in the same tumor. A common feature of these mutations is that they are all in- volved with signaling along the mitogen-activated protein kinase (MAPK) pathway. RAS mutations are uncommon in the follicular variant of papillary car- cinomas. The presence of PAX8-PPARg translocati- ons, RAS, CTNNB1 and p53 mutations in folllicular thyroid cancer may be a marker for progression to poorly differentiated and undifferentiated carci- noma. RET rearrangements have been identified in both heritable and sporadic medullary thyroid carcinoma.

Tiroid kanseri en yaygın endokrin malignitedir ve yıllık endokrin organlara ait kanser ölümlerinin ço- ğundan sorumludur. Tiroid follikül epitelinden kay- naklanan diferansiye tiroid kanserlerinde medüller tiroid kanserinde olduğu gibi germ line geçişi gös- terilmiştir; üzerinde detaylı çalışılmış bir mutasyon ya da sorumlu bir gen söz konusu değilse de tiroid kanserinde yine de genetik yatkınlıktan söz edebil- mek olasıdır. Olası birtakım tümör supressör genler ve onkojenler üzerinde durulmaktadır. Son 20 yıl- dır bu tümörlerle ilgili çalışmalarda, moleküler tek- nolojilerin uygulanması ile genetiğin spesifik tiroid tümörlerinin gelişimi ile ilişkisi aydınlatılmıştır. Papiller tiroid kanserinin % 70inde RET, TRK, BRAF, RAS gibi genetik mutasyonlar izlendiği halde bun- lar aynı tümörde eşzamanlı görülmez. Bu mutas- yonların ortak bir özelliği hepsinin mitojen-aktive edici protein kinaz yolağı ile ilgili olmalarıdır. RAS mutasyonları papiller tiroid kanserinin follikü- ler varyantında enderen görülür. Folliküler tiroid kanserindeki PAX8-PPARg translokasyonları, RAS, CTNNB1 ve p53 mutasyonları gibi genetik değişim- ler kötü diferansiye ve anaplastik (farklılaşmamış) tiroid kanserlerinin gelişimi ve progresyonunda da etkili olmuştur. RET mutasyonu hem ailevi hem sporadik medüller tiroid kanserinden sorumlu tu- tulmuştur.
