The safety and effectiveness of percutaneous nephrolit- hotomy in the patients with previous open renal surgery

Abstract

Objective: The aim of this study was to evaluate the effective- ness and safety of percutaneous nephrolithotomy (PNL) in the patients with previous open renal surgery. Material and Methods: 976 patients who underwent PNL between 2003-2013 were evaluated retrospectively. The pati- ents were divided into two groups based on whether they have previous surgery on the ipsilateral kidney or not. 160 patients (% 60 male, % 40 female; average age 41.7) who had pre- vious open renal surgery were categorized in group 1, whi- le 816 patients with no history of surgery (% 55 male, % 45 female; average age 38,6) took part in group 2. Two groups were compared in terms of stone burden, percutaneous access number, operation time, length of hospital stay, success rate and complications. Results: There was no significant difference in terms of age, gender and stone burden (p>0.05). The average operation time in group 1 was 77,7 and in group 2 was 75,1; the average per- cutaneous access number was 1.27 and 1.33 respectively and the average length of hospital stay was 3.0 and 2,9 respectively (p>0.05). The stone free rate in group 1 was % 86 and in group 2 was % 88 (p>0.05). The most common complication in both two groups was blood transfusion requiring bleeding (% 6,8 and % 6,3 respectively) followed by fever above 38°C (% 9.1 and % 11,2 respectively). There was no significant difference between two groups in terms of these complications (p>0.05). Conclusion: Percutaneous nephrolithotomy is a safe and ef- fective procedure that can be applied to the patients with previ- ous renal surgery as well as to the patients having no previous surgery.

Amaç: Bu çalışmada amacımız geçirilmiş böbrek cerrahisi olan hastaları perkütan nefrolitotomi (PNL) işleminin gü- venilirliği ve etkinliği açısından değerlendirmektir. Gereç ve Yöntem: 2003-2013 yılları arasında PNL ameli- yatı uygulanan 976 hasta geriye dönük olarak değerlendi- rildi. Hastalar daha önce aynı taraftan açık taş ameliyatı yapılıp yapılmadığına göre iki gruba ayrıldı. Grup 1’de daha önce açık cerrahi geçirmiş olan 160 hasta (% 60 er- kek, % 40 kadın; ort. yaş 41.7); grup 2’de ise daha önce ameliyat geçirmemiş olan 816 hasta (% 55 erkek, % 45 ka- dın; ort. yaş 38.6) yer aldı. İki grup taş yükü, perkütan giriş sayısı, ameliyat süresi, hastanede kalış süresi, başarı oranı ve komplikasyonlar açısından karşılaştırıldı. Bulgular: İki grup arasında yaş, cinsiyet ve ortalama taş yükü açısından anlamlı fark yoktu (p>0.05). Ameliyat süre- si grup 1’de ortalama 77.7 dk., grup 2’de 75.1 dk.; ortala- ma perkütan giriş sayısı sırasıyla 1.27 ve 1.33; hastanede kalış süresi ortalama 3.0 ve 2.9 gün bulundu (p>0.05). Taş- sızlık oranı grup 1’de % 86, grup 2’de % 88 olarak tespit edildi (p>0.05). İki grupta da en sık görülen komplikasyon kan nakli gerektiren kanama (sırasıyla % 6.8 ve % 6.3) idi; bunu yüksek ateş (>38°C) izlemekteydi (% 9.1 ve % 11.2). Her iki komplikasyon açısından iki grup arasında anlamlı fark yoktu (p>0.05). Sonuç: Perkütan Nefrolitotomi, geçirilmiş renal cerrahili hastalarda da primer hastalar kadar yüksek etkinlik ve gü- venlikle uygulanabilecek bir cerrahi prosedürdür.
